Follow-up to the echo-numbering fix: with narration unnumbered, a `failure` hung on an echo step — which the runner never produces, since echo only passes or skips — would head its block with the previous step's number, or with 0 before any real step has run. The CLI's `renderFailures` already drops them for exactly that reason; the MCP renderer now matches.
