## Known Issues

1. **Issue**: Agent templates may not be found if package not installed
   - **Status**: Handled with fallback to basic agent creation
   - **Workaround**: Install `agentic-qe` package

2. **Issue**: Permission errors on script files
   - **Status**: Fixed with `chmod 755` on scripts
   - **Workaround**: Manual `chmod` if needed

3. **Issue**: Directory exists errors on re-initialization
   - **Status**: Handled with `fs.ensureDir()` (idempotent)
   - **Workaround**: None needed
