Fixes #13 - Agent spawning error "Unknown agent type: performance-monitor"

Bug Fixes:
- Fixed agent type in Config.ts: performance-monitor → performance-tester
- Fixed env variable: PERFORMANCE_MONITOR_COUNT → PERFORMANCE_TESTER_COUNT
- Fixed README.md math error: 59 → 60 total skills (35 QE + 25 CF)

Documentation Updates:
- Added bug fixes section to CHANGELOG.md (v1.3.0)
- Added bug fixes section to RELEASE-NOTES-v1.3.0.md
- Updated GitHub issue #13 with resolution for v1.3.0
- Removed "README skill count update needed" from known limitations

Impact:
- Fleet now starts correctly without agent spawning errors
- All 18 QE agents initialize successfully
- Documentation accurately reflects skill counts
