🚨 CRITICAL BUG FIX - Emergency Patch Release

Root Cause:
- package.json contained self-dependency 'agentic-qe@^1.3.3'
- This caused npm to install BOTH v1.4.0 and v1.3.3
- Both versions registered 54 MCP tools with identical names
- Result: API Error 400 - 'tools: Tool names must be unique'

Impact:
- ALL 18 QE agents failed to spawn in v1.4.0
- Affected all users who installed v1.4.0 from npm

Fixes Applied:
1. Removed self-dependency from package.json dependencies
2. Updated package.json 'files' array to explicitly include only:
   - .claude/agents
   - .claude/skills
   - .claude/commands
3. Added .claude/settings*.json to .npmignore

Documentation Updates:
- package.json: 1.4.0 → 1.4.1
- CHANGELOG.md: Added v1.4.1 emergency patch section
- README.md: Updated version and Recent Changes with critical fix notice

Verification:
- Removed node_modules/agentic-qe directory
- Verified settings.json excluded from package
- Build successful with v1.4.1
- Package size: 2.2 MB, 1672 files
