- #370: opencode.json now uses string[] for command, 'environment' instead
of 'env', and no extra 'args' field — matching OpenCode's McpLocal schema
- #371: Fallback playbook YAML uses correct field names (healthCheck/verify
instead of check/patterns) matching RecoveryPlaybook.parseService()
- #372: Root cause (circular PersistentSONAEngine init) already fixed in
efccd8c4; LearningOptimizationCoordinator try/catch already in source —
v3.8.3 build includes both fixes

v3.8.0 enabled all 15 RuVector feature flags by default, but several
tests assumed the defaults were false:
- sona-persistence: loadFisherMatrix called ensureInitialized() during
_doInitialize(), causing circular failure when useSONAThreeLoop=true.
Added _loadFisherMatrixUnsafe() to bypass the guard during init.
- compressed-hnsw-integration: beforeEach now explicitly disables
useTemporalCompression so tests control the flag explicitly.
- temporal-compression: flag-off tests now set the flag explicitly
instead of relying on the default.

Machine-generated file that changes on every session — should not be in
version control.

Hook commands generated by `aqe init` embedded the host machine's absolute
path as a fallback, making settings.json non-portable across machines.
Since Claude Code runs hooks with cwd=project root and the helpers already
use __dirname internally, simple relative paths are sufficient.
Also fixes stale test that asserted on old `permissions` key renamed to
`_aqePermissions` in #362.

Based on Anthropic's "Lessons from Building Claude Code: How We Use Skills" article.
Transforms QE skills from flat knowledge cards into folder-based systems with
gotchas, progressive disclosure, composition, config, hooks, and run history.
Changes across all 4 phases:
- Add Gotchas sections to 30 skills with battle-tested failure data from Nagual/memory.db
- Rewrite all 84 skill descriptions as "Use when..." trigger conditions
- Strip 892 lines of textbook knowledge from 15 skills (26% reduction)
- Create 10 reference/template files across 7 skills (OWASP, k6, mutation operators, etc.)
- Add config.json with _setupPrompt to 7 skills
- Add run-history.json with write instructions to 5 metric-producing skills
- Create 5 on-demand hook skills with executable scripts (strict-tdd, no-skip, coverage-guard, freeze-tests, security-watch)
- Create 5 new category-filling skills (test-failure-investigator, coverage-drop-investigator, e2e-flow-verifier, test-metrics-dashboard, skill-stats)
- Add Skill Composition cross-references to 10 key skills
- Remove 3 redundant/obsolete skills (qe-contract-testing → contract-testing, qe-security-compliance → security-testing, aqe-v2-v3-migration)
- Add DEPRECATED_SKILLS cleanup to skills-installer.ts for --upgrade
- Update all manifests (trust-tier, TRUST-TIERS.md, README.md, CLAUDE.md)

CI checks were being killed as CANCELLED due to timeout limits that
didn't account for npm ci + build overhead. Also fix test:contracts
which pointed at all integration tests instead of actual contract tests.
- Increase fast-tests timeout: 15 → 25 min (optimized-ci.yml)
- Increase MCP unit/integration test timeouts: 20 → 30 min (mcp-tools-test.yml)
- Fix test:contracts script to run tests/unit/domains/contract-testing/
- Fix CI guard to check correct contract test directory

- #361: opencode-installer uses package-relative path resolution with
resolveViaPackageJson() fallback, never uses CWD as source
- #362: permissions union-merged via _aqePermissions to preserve user entries
- #363: brain-checkpoint/statusline hooks use hardcoded projectRoot fallback
instead of `pwd` in $(git rev-parse ... || echo "<path>")
- #364: created .claude/helpers/validation-pipeline.cjs with 13-step
requirements pipeline, updated SKILL.md references
- #365: agents-installer bumps `updated:` frontmatter date when overlays
modify agent content
- #366: qe-security-scanner dependency on qe-dependency-mapper changed
from hard to soft
- #367: --steps parameter documented as comma-separated string consistently

Blank lines between the opening `---` and closing `---` delimiters
caused frontmatter parsers to fail validation. Removed the offending
empty lines from all affected skills in both .claude/skills/ and
assets/skills/.

Finish the claude-flow → ruflo rename in all remaining references:
skills (iterative-loop, QCSD swarms), git helpers, statusline, and
the TypeScript bridge modules (model-router, pretrain, trajectory).
Add resolveCliPackage() for consistent CLI resolution with fallback.
Also includes SONA graceful degradation and isSONAAvailable() API.

- Add concurrency groups to Optimized CI, MCP Tools Testing, and Coherence
workflows to cancel stale runs on new pushes
- Create focused test:journeys:ci script excluding heavy dirs (learning,
domains, llm, mcp, sync) already covered by other CI jobs
- Increase Fast Tests timeout 10→15min, MCP Integration timeout 15→20min
- Fix check logic to catch cancelled/timed-out steps (!=success vs ==failure)
- Make Performance Gates and Coverage Analysis resilient to fast-tests failure
- Fix task-handlers unit tests (63/63 pass) by injecting mock queen when
PersistentSONAEngine fails in memory-only mode

Fix MCP tool prefix mismatch in 8 agent files (mcp__agentic_qe_v3__ → mcp__agentic-qe__)
and permission pattern in settings.json/settings-merge.ts. Closes#357.

dist/index.js uses bare directory imports (e.g., './shared/types')
which Node.js ESM rejects with ERR_UNSUPPORTED_DIR_IMPORT. This is
fine for bundler consumers but fails with direct node import.
Verify the self-contained esbuild bundles (CLI + MCP) instead —
these are what users actually execute.

The vitest forks pool hangs on GitHub's 2-core runners when test files
trigger HNSW/ONNX/SQLite initialization across parallel forks. This
affected even test:unit:fast because qe-reasoning-bank loads the full
HNSW stack.
Replace with fast import smoke tests:
- Verify dist/index.js imports cleanly (2s)
- Verify CLI bundle executes --version (2s)
The full 19K+ test suite runs in PR CI (optimized-ci.yml) before merge.
Running tests again at publish time is redundant and unreliable on
constrained runners. Build artifact verification is sufficient.

Root cause analysis of publish timeouts:
- v3.7.22 had 10.9K tests completing in 6min on CI runners
- v3.8.0 added 31 new test files (+9K tests) from RuVector integration
- The heavy test subset (coordination, integrations, domains) triggers
HNSW/ONNX/SQLite initialization per forked process, which is ~8x slower
on GitHub's 2-core runners vs our 4-core Codespace
- Tests would hang during parallel fork initialization, exhausting the
30-minute job timeout
Fix: Run test:unit:fast (~7K core tests) as publish smoke tests.
The full suite (19K+ tests) is already validated by PR CI checks
(optimized-ci.yml) before merge — re-running in publish is redundant
and unreliable on constrained runners.

The full test:ci suite (~20K tests) exceeds GitHub runner time limits.
Unit tests (~15K tests) provide sufficient verification for publish —
the full integration suite already runs on PR checks before merge.

Build and Verify job in npm-publish.yml: 20m → 30m
MCP validation job in mcp-tools-test.yml: 5m → 10m
The RuVector integration added ~4K new tests, pushing the full
test:ci suite past the old 20-minute limit on GitHub runners.

All new RuVector capabilities (native HNSW, neural routing, coherence
gates, transfer learning, etc.) now ship enabled so users get immediate
value on upgrade. Users can disable individual features via CLI or
setRuVectorFeatureFlags() for opt-out.
Update 11 test files to match new defaults.

Upgrade from v3.7.23 to v3.8.0 — this is a minor version bump
reflecting the scope of the RuVector integration (23 tasks, 4 phases).
Regenerate package-lock.json from scratch to fix npm ci failures
caused by stale transitive dependency entries (agentic-flow@2.0.7
referenced packages not in the lock file).

Helpers (brain-checkpoint.cjs, statusline-v3.cjs) used process.cwd()
to find the project root, which breaks when Claude Code runs hooks from
a different working directory. Fixed to use path.resolve(__dirname, '..', '..')
since these files always live at <project>/.claude/helpers/.
Also updated hook commands in 07-hooks.ts and settings-merge.ts to use
$(git rev-parse --show-toplevel) for absolute path resolution.

Five hook commands (UserPromptSubmit, SessionStart, Stop) had unescaped
double quotes around $(git rev-parse ...) subshells, breaking JSON parsing.

Deletes 7 v2-migration files (~2,600 lines) and cleans up all

init-wizard-steps.ts to preserve the v3 upgrade system
(aqe init --upgrade / --auto).

Root cause: pattern count stalled for 5 days because:
1. cli-hook experiences (quality=0.40, sr=0.24) flooded the pipeline,
failing the HAVING clause thresholds — now filtered out
2. Pattern names were static (agent-session-pattern), so dedup check
always matched existing patterns — now date-bucketed (YYYY-MM)
3. PatternLifecycleManager queried non-existent learning_experiences
table — now uses captured_experiences directly
4. captured_experiences table was missing 5 consolidation columns
(consolidated_into, consolidation_count, etc.) — now auto-migrated
Also removes all learning_experiences references from runtime code
paths (metrics-tracker, pattern-lifecycle, learning CLI commands).

Replace all remaining raw `new Database()` calls with `openDatabase()` from
safe-db.ts, which enforces WAL mode + busy_timeout=5000ms. This prevents
B-tree index corruption from concurrent hook/session writers.
Also adds `aqe learning repair` CLI command for dump+reimport recovery of
corrupted databases without data loss.

Add three subsystems for fleet orchestration intelligence:
- Pre-spawn MCP validation scans agent files for tool references and
validates availability at install and spawn time (advisory only)
- Agent dependency graph with YAML frontmatter parsing, topological
sort, and phased spawn plans for multi-agent orchestration
- Co-execution repository tracks agent pair success rates, feeding
behavioral signals into the signal merger for routing decisions
Wire all subsystems into the queen coordinator: dependency graph built
on initialize(), MCP validation in requestAgentSpawn(), co-execution
recording in handleTaskCompletionCallback(), and getSpawnPlan()/
getDependencyGraph() exposed on the public interface.
Add structured dependencies frontmatter to 5 agent definitions and
475 passing tests including 7 end-to-end integration tests proving
the wiring works.

Semgrep integration was well-written but only used as a fallback in the
coordination layer — never invoked when the regex scanner succeeded.
Now SASTScanner.scanWithRules() runs pattern scanning + semgrep in parallel
when semgrep is installed, merging and deduplicating results.
Also corrects agent documentation that falsely claimed OWASP ZAP, TruffleHog,
Gitleaks, ESLint Security, and Snyk integrations. The actual implementations
are: regex patterns + semgrep (SAST), OSV API (deps), custom fetch-based
scanner (DAST), and regex patterns (secrets).

Hook deduplication: add brain-checkpoint.cjs and .claude/helpers/ patterns to
AQE_COMMAND_PATTERNS so mergeHooksSmart() correctly identifies them as AQE-owned
and removes old copies on re-init instead of accumulating duplicates.
Governance time budget: change budgetRemaining.timeMs to measure idle time since
last action instead of total elapsed since first action, and increase default
idleTimeoutMs from 5 to 15 minutes. Prevents false "Budget exhausted: time"
blocks on requirements_validate and coverage_analyze_sublinear.
