Instead of manual file editing, EU and self-hosted users can now set
the POSTHOG_MCP_URL environment variable. This follows the pattern
used by other MCP servers like Postman and GitHub.
- Use template variable in .mcp.json: ${POSTHOG_MCP_URL:-default}
- Update README with env var instructions for EU Cloud
- Unify self-hosted config to use same env var
- Remove unnecessary .mcp-eu.json file

The `?region=eu` query parameter does not work due to a known Claude Code
OAuth issue where it ignores the authorization_servers field from OAuth
metadata. EU users must use the mcp-eu.posthog.com subdomain instead.
Changes:
- Add .mcp-eu.json config file for EU Cloud users
- Update README with correct EU setup instructions
- Link to the Claude Code issue for context
Fixes the issue reported by Kristo in the docs thread.

- EU Cloud: clarify that region=eu param is recommended (auto-detect
works but param is faster/more reliable)
- Remove "Coming soon" section - all commands are now available
- Add missing commands to list (logs, llm-analytics, docs, actions,
search, workspace)
