Migrate to React Router with path-based routes

Replace manual URL manipulation (pushState, popstate, URLSearchParams)
with React Router's createBrowserRouter. Routes: /diff, /tree, /tour/:id
with / redirecting to /diff. Loaders prefetch data before route
transitions. CLI URLs updated to match new paths.
Removes: app.tsx, hooks/use-search-params.ts
Adds: root-layout, loaders (diff, tree, tour), tour-tree-page wrapper

Introduces a tour system that lets agents create narrated, step-by-step
code walkthroughs. Each tour has a topic, intro body, and ordered steps
pointing to specific file:line ranges with narrative text and annotations.
CLI: tour-start, tour-step, tour-done agent subcommands
API: CRUD endpoints for tours and steps
DB: tours and tour_steps tables
UI: right-side tour panel with step navigation, file highlighting,
and inline annotations

fix: comment navigation, sidebar filter, and path comment counts
...
- Fix thread navigation: resolve __path__: prefixed filePaths to
actual paths, determine file vs dir, await ensureQueryData before
scrolling instead of fragile setTimeout
- Fix sidebar comment counts: translate __path__:foo back to foo
so file-level comments show the indicator badge
- Fix comment filter: same translation ensures files with path-level
comments appear when filter is active
- Add data-thread-id to path comment cards for scroll targeting
