feat(ce-simplify-code): report quantified impact by dimension

Add an impact measure to the Step 5 summary: fixes applied per reviewer
dimension (reuse/quality/efficiency), findings skipped, and the
behavior-preservation result. Because the skill applies its fixes, this is a
real post-hoc account of what improved — deliberately not a net-lines-removed
scoreboard, which would mismeasure the many clarity, safety, and efficiency
fixes that preserve or add lines and would fight the skill's
comprehension-over-compactness goal.

feat(ce-code-review): report removable-surface signal in Coverage

Surface a single Coverage line estimating the net lines/files that
deletion-oriented maintainability findings would remove if applied, giving
reviewers a cheap read on how much dead weight a diff carries. Framed as a
signal, never a reduction target, and omitted when no finding proposes a
deletion.

fix(ce-simplify-code): guard safety checks and catch stdlib reinventions

Add a hard rule that simplification never strips input validation, data-loss
error handling, security checks, or accessibility affordances — closing a gap
where these could be removed as apparent boilerplate while still "preserving
behavior" on scoped tests.
Extend the reuse reviewer to flag diff code that reimplements a language
stdlib/runtime primitive, gated on behavior-equivalence and excluding
UX-changing swaps.
