docs(solutions): refresh model-normalization doc for sonnet-5/opus-4-8

The learning's current-state examples still showed the pre-bump alias
values (claude-sonnet-4-6 / claude-opus-4-6) as "the single source of
truth" — stale since those aliases moved to claude-sonnet-5 /
claude-opus-4-8. Update the solution code block, behavior table, and
rationale prose to the live values, add a note that src/utils/model.ts
holds the canonical map (so the illustration won't silently re-drift each
generation), and cross-link the companion sampling-param learning.
Historical incident descriptions and the dated research snapshot are left
untouched — refreshing those would rewrite history, not fix drift.
