fix(ce-code-review): defer @-inlined references to on-demand loads

Five references (persona-catalog, subagent-template, diff-scope,
findings-schema, action-class-rubric) were @-inlined at the bottom of
SKILL.md, loading ~466 lines into every session at trigger time even
though all are late-sequence (needed only at Stage 3/4). This violated
the repo's own AGENTS.md extraction rule ("Never use @ for an extracted
block"); the skill already deferred two analogous references on demand.
Convert all five to on-demand loads with load-bearing pointers at the
consuming stage, cutting the always-loaded footprint by ~473 lines.
Also fix the persona count (14 -> 13 structured reviewers) in SKILL.md
and persona-catalog.md, and a stale artifact-path reference in
subagent-template.md (.context/ -> /tmp/compound-engineering/...).
Validated with four dry-run dispatches across selection paths (lite,
full+conditionals, mode:agent, stack-specific): all four read the
deferred references and produced byte-identical schema/diff-scope and
verbatim catalog spawn-gate content, confirming the on-demand loads
fire reliably.
