fix(cross-model): give the heartbeat its own process group
...
Callers restore set +m before start_heartbeat, so the heartbeat inherited
the worker pgid and kill -- -PID could not reach the inner sleep. Enable
job control around the heartbeat spawn so group teardown actually works.

fix(cross-model): unblock heartbeat teardown on SIGTERM
...
Bash defers a leader-only TERM until the inner wait on sleep returns, so
stop_heartbeat could block for the 60s interval and trip CI's 20s timeout.
Signal the heartbeat process group so the sleep child dies and wait returns.

jq // empty plus a redirect left a zero-byte usage file when the Codex
stream had no turn.completed, so downstream json.load failed. Keep the
artifact only when it actually contains usage JSON.
