fix(ce-commit-push-pr): elevate multi-PR descriptions and slim skill prose

Add bidirectional program altitude (lead-in/lead-out) and scoped STE
framing so multi-PR work places each PR on the arc, then compress
duplicated judgment bulk so the skill surface shrinks while keeping
the same outcomes.
