Claude d95ea2a11c Separate label and comment idempotency ...

The label should always be re-applied on synchronize events (new commits
after review mean re-review is needed), but the welcome comment should
stay idempotent. Move the label/createLabel logic before the marker
check so `review-pending` is ensured on every run.
