fix: add .DS_Store to gitignore, add swift plugin.json and allowed-tools

- Remove committed .DS_Store files and add to .gitignore
- Add missing .claude-plugin/plugin.json for swift skill (consistency with other skills)
- Add allowed-tools: WebFetch to swift SKILL.md frontmatter (skill requires web access for /v1/environment and quickstart)
