fix: add safe default mock for fetch in fetch-bug-versions tests ( #145 )

The beforeEach spy on globalThis.fetch had no default mock implementation,
meaning if fetchWithTimeout or fetchFirstJson was called before per-test
mocks were set up, it would hit real network endpoints. Per CLAUDE.md:
'Network-calling functions must be mocked to prevent real HTTP requests.'
Now the spy is initialized with mockRejectedValue to block all real
network calls by default. Individual tests override with specific mocks
as before.
