hono <4.12.25 is vulnerable to CVE-2026-54290 (GHSA-88fw-hqm2-52qc):
the CORS middleware reflects any request Origin with
Access-Control-Allow-Credentials: true when credentials are enabled and
origin is left at the default wildcard, exposing cookie-authenticated
endpoints to arbitrary origins.
- packages/world-testing: hono 4.12.21 -> 4.12.25 (the flagged manifest)
- workbench/hono: ^4.12.8 -> ^4.12.25, clearing the also-vulnerable
4.12.9 from the lockfile
Neither app uses hono's CORS middleware, so neither was exploitable, but
the bump clears the vulnerable code from the dependency tree. Only the
core Hono class is imported in world-testing; build and typecheck pass.

* Disable Nuxt telemetry via workspace .nuxtrc instead of inline env vars
The inline NUXT_TELEMETRY_DISABLED=1 in package.json scripts doesn't work
on Windows (pnpm runs scripts through cmd.exe). A workspace-root .nuxtrc
is cross-platform, and @nuxt/kit resolves it from the pnpm workspace root,
so it also covers workbench/nuxt's build/dev, which hit the same
first-run consent prompt.

The test server's flow handler awaited flowPOST before incrementing the
invocation counter, which races with the test polling getRun() to see
the completed status. When the workflow completed inside flowPOST and
flushed the run to the DB, the test could observe the completed state
and immediately query /_flow-invocations before the counter was bumped,
yielding a flaky 'expected 0 to be 1' assertion.
Increment the counter before awaiting flowPOST so the count is
observable as soon as the run transitions to completed.
The previous pre-release versions (4.x.y-beta.N) caused two issues:
- semver.inc('4.0.0-beta.N', 'major') returns 4.0.0, not 5.0.0
- Pre-release numbers carried over (beta.61 -> beta.62 instead of beta.0)
Setting all versions to 4.0.0 (non-pre-release) ensures a clean major
bump to 5.0.0-beta.0. Also removes @workflow/swc-playground-wasm from
the changeset and pre.json since it is a private package.

Add missing error exports (HookNotFoundError, EntityConflictError,
RunExpiredError, TooEarlyError, ThrottleError, RunNotSupportedError,
WorkflowWorldError) to workflow/internal/errors. Create new error
classes for world-level semantics. Tighten TSDoc comments on all
error classes. Add API reference docs for all error types.
