CE Lin 1ce59d484a fix(context): honor CLAUDE_MEM_CONTEXT_OBSERVATION_TYPES on session-start injection (#3410 )

CLAUDE_MEM_CONTEXT_OBSERVATION_TYPES and _CONCEPTS are documented and
persisted by SettingsRoutes, but never reached the injection query: the two
keys were missing from SettingsDefaultsManager.DEFAULTS so loadFromFile
dropped them, and loadContextConfig built both sets from the active mode file
alone. A user narrowing their injected context got the mode's full list.

Both keys are now declared with an empty default, and loadContextConfig parses
them as comma-separated lists, tolerating whitespace and empty entries, and
falls back to the mode-wide lists when empty. Five tests cover both halves of
the defect.

Verified on merge of origin/main (c340932c2): bun test tests -> 3721 pass,
28 skip, 2 fail (both pre-existing on main: field-deadline-wire real-network
test and plugin-distribution npm-tarball test that needs a build). tsc
--noEmit clean.
