Closes #79

## Added files (vscode/)
- copilot-instructions{,-en,-ja}.md — global workspace instructions (auto-active)
- instructions/pua{,-en,-ja}.instructions.md — path-level instructions with applyTo: "**"
- prompts/pua{,-en,-ja}.prompt.md — /pua slash command for Copilot Chat

## Fixes in prompt frontmatter
- Replace deprecated `mode: agent` with `agent: 'agent'`

## README updates (all 3 languages)
- Add VSCode installation section with 3 methods
- Fix English README to reference -en variant files (was incorrectly using Chinese files)
- Correct required settings per method:
  - Method 1: github.copilot.chat.codeGeneration.useInstructionFiles
  - Method 2: chat.includeApplyingInstructions
  - Method 3: no settings needed
