The bundled agents/ payload shipped references/quality-control-checklist.md alongside the agent definition, and the installer copies that tree verbatim into every editor's agents directory. OpenCode lists every markdown file it finds there, so the checklist showed up as a bogus 'References/Quality-Control- Checklist' agent and came back on every update (#948). The checklist describes the mandatory quality_control output field, so the agent needs it on every run anyway — and a spawned agent cannot resolve the relative path on a global install.
