A system alert leaves the app foregrounded, so the crawler maps that screen - and SpringBoard's status bar is in the AX tree alongside it. The clock is the one status-bar item shaped like a title row (0.117 wide against the 0.07-and-under carrier and battery readouts) and it sits above the nav bar, so it anchored the title band on any screen whose own title carries no header-ish role. A crawl of Settings titled a screen "16:41".
