ruv ce1b33d2de feat(swarm): ADR-069 no-key honest degradation for swarm --execute

Coordination follow-up: when EVERY worker fails on an auth/credit error
(looksLikeAuthError), the overview degrades honestly — next steps state no
usable model auth was detected and point at the defer path (claude -p /
hive-mind spawn --claude / --no-execute plan-only), per ADR-385 consequences.

Detection runs AFTER execution, not as a pre-flight gate: ambient `claude -p`
auth has no single env signal, so gating on an env var would wrongly fall back
even where auth works (as the e2e proved). Where auth is present, execution is
unchanged. +2 unit tests (fallback fires on all-auth-fail; does not fire when
any worker succeeds). State writes keep the existing `swarmId` key — the
status/stop id reconciliation (#3253) is untouched.
