feat(hooks): V3 ReasoningBank guidance system with CLI
- Implement ReasoningBank with AgentDB vector storage (no JSON)
- Add GuidanceProvider for Claude-visible output generation
- Create CLI entry point for hooks guidance system
- Add security checks: block .env files, dangerous commands
- Add agent routing with historical performance tracking
- Build TypeScript and verify compilation
