rUv e111bfbd7f fix(memory,tests): restore memory search recall and green the CLI suite (69 → 0) (#3252)
* fix(memory,tests): restore `memory search` recall and green the CLI suite (69 → 0)

The CLI suite had 69 failing tests across 50 files. Root-causing each rather
than adjusting expectations turned up one real product regression, two real
bugs, and a set of tests asserting things about the machine instead of the code.

PRODUCT REGRESSION — `memory search` recalled nothing (re-break of #2558)

`memory search` returned zero results for content that matched word for word,
while store/list/retrieve worked. Bisected to the threshold default:

  * 2026-07-04 #2558 restored recall, designed around a 0.3 threshold. Its
    fusion scores a full-coverage keyword hit as
    0.6*max(0,semantic) + 0.4*lexical, so with a non-positive cosine — routine
    for a one-word query — a PERFECT keyword match tops out at exactly 0.40.
  * 2026-07-26 #2790 set the CLI default to 0.7. Above 0.4, keyword recall is
    mathematically unreachable, so #2558 silently regressed.

Instrumented the live path to confirm before changing anything:
  key=note/alpha terms=["connectivity"] cov=1 bm25=0.029 sem=-0.883
  lex=1.000 score=0.400 thr=0.7      → dropped
Default restored to 0.3 at both sites, with the ceiling documented so it is not
raised past 0.4 again. Verified end to end: a shared keyword recalls all three
entries, a unique keyword recalls only its own. This was broken on main too.

REAL BUG — anchor containment rejected any project under a symlink

containedPath() compared a realpath'd root against a NON-realpath'd candidate,
so on macOS (/tmp → /private/tmp) every project looked like an escape. Now
compares like with like. Both guarantees re-verified by probe: ../ traversal,
deep traversal, absolute-outside and symlink-escape all still rejected;
symlinked-root spelling now accepted.

REAL BUG — a test that could not fail

funnel.test.ts asserted `elapsed < 100ms` as a proxy for "no network call" —
its own comment conceded a future fetch() would still pass. It also failed
intermittently (118ms) under full-suite CPU contention. Replaced with an
actual assertion on fetch/http.request/https.request, patched via the CJS
copies (ESM namespaces are non-writable). Negative-controlled: the technique
observes a real call and restores cleanly.

TESTS THAT MEASURED THE MACHINE, NOT THE CODE

  * 51 wasm tests asserted `available === true` for optional packages that were
    simply not installed, gated only on `process.env.CI`. Now also gated on real
    availability. The 7 tests in those files that need no WASM were lifted into
    their own blocks so they keep running rather than being skipped along with
    the rest — no coverage traded away for a green tick.
  * 4 agenticow tests lacked the `skipIf(!havePkg)` guard their 4 siblings had;
    in degraded mode the verb is inert and returns before validation.
  * policy-runtime hashed the raw tmpdir path while the source hashes the
    CANONICAL one, so it looked for a trust anchor that is never written there —
    and its cleanup was deleting a nonexistent path, leaking real anchors into
    ~/.config on every run.
  * sona's provider allowlist predated the ruvector / wasm-embedder /
    @claude-flow/embeddings backends.
  * ruvector/index asserted vi.mock satisfies a *dynamic* import() of a bare
    specifier. It does not under vitest 4 — the specifier is rewritten to
    '/@id/@ruvector/core' and fails with ERR_MODULE_NOT_FOUND. Now pins the
    contract the try/catch actually provides: unloadable → false, never throws.

TIMEOUTS — the "flakiness" was a fixed 5s budget

Six memory/intelligence files failed with "Test timed out in 5000ms", never an
assertion. They initialise a real ONNX embedder and SQLite; alone they are
fast, but the full suite saturates every core (~440% CPU). Because it depended
on scheduling, a different file failed each run. Raised testTimeout/hookTimeout
to 30s. (Process-level isolation was tried first and ruled out — forks changed
nothing, which is what identified the cause as time, not shared state.)

Verified: 3 consecutive full runs at 0 failed / 3572 passed on the feature
base, and 0 failed / 3537 passed on this branch's base.

* fix(tests): make ruvector availability assertion environment-independent

CI caught what my first pass missed. The rewritten test asserted
`isRuvectorAvailable()` resolves to `false`, but whether the dynamic
`import('@ruvector/core')` resolves is environment-dependent:

  - under vitest's module runner a bare-specifier dynamic import is
    rewritten to '/@id/@ruvector/core' and fails (ERR_MODULE_NOT_FOUND)
    → false → my `toBe(false)` passed locally
  - a normal install where the package resolves → true → `toBe(false)`
    fails, which is exactly what the CI test-ratchet flagged

The prior `toBe(true)` had the same defect in the other direction. Pin
the invariant that holds in BOTH environments instead — the only thing
the try/catch actually promises: it resolves to a boolean and never
throws. No specific resolution outcome is asserted.

Verified 33/33 locally; the sibling "should return boolean" test already
covers the type, so this now documents the no-throw contract distinctly.

rUv 07f1cf3a3f fix(agentbbs-tools): detect the real CLI-only agentbbs package instead of a nonexistent JS import (#3250)
* fix(agentbbs-tools): detect the real CLI-only agentbbs package instead of a nonexistent JS import

The published `agentbbs` package (0.2.1) is a CLI-only launcher — a `bin`
script with no importable JS entry point — that downloads/builds a Rust
binary and shells out to it. loadAgentbbs()'s `await import('agentbbs')`
could therefore never resolve, even with the CLI correctly installed and
on PATH, so all 4 federation_bbs_* tools reported degraded:true
unconditionally regardless of install state.

Replace the dynamic import with a subprocess presence probe
(agentbbsCliAvailable(), `agentbbs --version`), matching how the rest of
the codebase treats optional CLI tools. shell:true is required for the
probe to find npm's Windows .cmd shim (child_process does not consult
PATHEXT otherwise).

Making the probe actually succeed surfaced two further pre-existing bugs
in code that was previously always skipped:
- vitest.config.ts externalized @noble/ed25519 alongside genuinely-optional
  deps (agentic-flow, agentdb, ...), but it is an always-installed hard
  dependency; externalizing it broke dynamic-import resolution under Vite's
  SSR transform.
- federation_bbs_human_join dynamically imported @noble/ed25519 a second
  time (getSigningKey() already does), which is now deduplicated behind one
  memoized loadEd25519() loader.

All 12 tests in agentbbs-tools.test.ts now pass, including the 4
happy-path tests that were previously always skipped.

* fix(agentbbs-tools): gate the probe shell to win32 instead of always on

`shell: true` was unconditional, and the probed binary comes from
`process.env.AGENTBBS_BIN`. Under a shell that argument is command-interpreted,
so `AGENTBBS_BIN='agentbbs --version; touch /tmp/PWNED'` executes the trailing
command. Verified locally: the file is created with `shell: true` and the spawn
fails with ENOENT once the shell is gated off.

This is the case browser-tools.ts already warns about in the comment beside the
same pattern -- "if user-controlled args are ever added, escape them before
spawn" -- and `shell: process.platform === 'win32'` is the convention already
used at browser-tools.ts:49, commands/init.ts:856 and :1194, and
init/helpers-generator.ts:1459. Unconditional `shell: true` appeared nowhere
else in the package.

The Windows behaviour the original was reaching for is unchanged: cmd.exe still
resolves the `agentbbs.cmd` shim via PATHEXT. POSIX simply stops interpreting
the override. Added windowsHide to match the sibling call sites.

Verified the fix still does what it set out to do: with the CLI resolvable the
suite is 12/12 and the four previously-always-skipped happy-path tests execute,
with the shell disabled on POSIX.

Dragan Spiridonov 0e957da3a4 fix(memory): migrate ADR-323 provenance_type on the bridge path, and stop the swallowed error surfacing as a WAL refusal (#2844)
* fix(memory): make a latched bridge-init failure diagnosable and recoverable

getRegistry() sets `bridgeAvailable = false` from a bare `catch {}`, and the
guard at the top of the function short-circuits every later call. One transient
init failure therefore routes every subsequent write to the sql.js whole-image
fallback for the remaining life of the process.

That fallback refuses whenever -wal/-shm sidecars are present, so the operator
sees only:

  memory database has an active native WAL connection (found -wal/-shm sidecar
  files) — refusing an unsafe sql.js whole-image write. Retry once the native
  writer completes, or restore the native better-sqlite3 bridge.

which names a cause they cannot check. Observed in the wild: an MCP server
silently dropped every memory_store for hours while CLI writes to the same
database succeeded. better-sqlite3 was healthy throughout — it loaded from
every resolution context and opened the database in WAL mode. The sidecars that
trigger the refusal are created by the healthy native writers, so the better
the native path works elsewhere, the more reliably a latched process fails.

Three changes:

- Record the init error instead of discarding it, and expose it through
  `getBridgeFailureReason()`. The WAL refusal now appends
  "Bridge unavailable: <reason>" at all three sites, via one helper rather
  than three copies of the literal.

- `shutdownBridge()` clears the cached state unconditionally. The reset used to
  sit inside `if (registryInstance)`, which is null in exactly the case that
  needs clearing — a failed init — so a latched process had no recovery short
  of a restart.

- Stop swallowing degradation notices during init. The suppressor filtered any
  message containing 'better-sqlite3' or '[AgentDB]', which matched AgentDB's
  own "better-sqlite3 not available, using sql.js WASM" line. The one message
  explaining why the native driver was not in use was being discarded as noise.
  Extracted as `shouldSuppressInitLog()` so it is testable.

ADR-053 (Implemented, 2026-02-25) documents the sql.js fallback as intentional
and is unchanged by this: the fallback still exists and still degrades the same
way — it is now observable and clearable, so no ADR update is required.

@claude-flow/memory joins the externalized optional deps in vitest.config.ts;
memory-bridge dynamic-imports it inside try/catch and its dist is absent unless
the workspace was built.

Verification: new suite 9/9 green, 7/9 fail against pristine. WAL-guard,
provenance, upsert and durability suites report 11 failed / 14 passed both with
and without this change — identical, pre-existing, none in the touched files.
tsc --noEmit: 437 errors on this branch and 437 on pristine main.

* fix(memory): migrate provenance_type on the bridge path (root cause of silent write loss)

The real defect behind the misleading WAL-sidecar refusal.

ADR-323 added `provenance_type` to bridgeStoreEntry()'s INSERT.
`ensureSchemaColumns()` backfills that column on the sql.js path, but the
bridge path only ran `CREATE TABLE IF NOT EXISTS memory_entries (...)` — a
no-op on a table that already exists. Any database created before ADR-323
therefore has no `provenance_type`, and every bridge write throws:

  SqliteError: table memory_entries has no column named provenance_type

The catch at the end of bridgeStoreEntry() returns null on anything that is
not a UNIQUE violation, so the error is discarded and the caller demotes to
the sql.js whole-image path — which refuses whenever -wal/-shm sidecars are
present, reporting "active native WAL connection … restore the native
better-sqlite3 bridge". That names a cause with nothing to do with the
failure. better-sqlite3 was healthy throughout.

Measured on a real install: `.swarm/memory.db` HAS provenance_type (the
sql.js path migrated it); `.swarm/agentdb-memory.db`, which the bridge writes
per #2786, does NOT. Every MCP memory_store was silently lost for hours while
CLI writes to the other database succeeded. Adding the column alone — with no
code change — turned bridgeStoreEntry from null into
`{success: true, embedding: {dimensions: 384}}`.

Changes:

- `provenance_type` is in the CREATE TABLE DDL (fresh databases) AND applied
  as an idempotent ALTER (pre-existing ones). SQLite has no ADD COLUMN IF NOT
  EXISTS, so attempt-and-ignore is the idiom.

- The schema block is extracted as exported `ensureBridgeSchema()`. The bug
  was untestable in place: the failure it causes is swallowed and resurfaces
  as an unrelated message, so nothing end-to-end could have caught it.

- bridgeStoreEntry's catch now records the swallowed error in
  `bridgeFailureReason`, so the WAL refusal carries the real cause.
  `getBridgeFailureReason()` is no longer gated on `bridgeAvailable === false`
  — a bridge that initialised fine can still fail every write, which is
  precisely this bug, and that case is the one worth reporting.

Verification: 5 new migration tests green; 3 of the 5 fail with the exact
production error when the ALTER is removed. Combined suites report the same 11
pre-existing failures as pristine, with 28 passing (14 of them new).
tsc --noEmit: 437 on this branch, 437 on pristine main.

rUv 495a451cf6 feat: add ADR-324 agentic policy engine (#2822)
* feat(memory): typed provenance in AgentDB (ADR-323)

Corrects and implements the 2026-07-28 dream-cycle research proposal
(PR #2804, closed as superseded) for typed memory provenance in
AgentDB, addressing MemIR/MemSyco-Bench's "provenance-role collapse"
and memory-induced sycophancy findings.

The dream-cycle PR's proposed schema change targeted the wrong table
(`vector_indexes`, which is per-namespace HNSW index metadata, not
per-entry) and its filename collided with the already-merged
`ADR-322-metaharness-flywheel-integration.md` (#2817) — both corrected
here as ADR-323 against `memory_entries`, the real per-record table.

- memory-initializer.ts: `provenance_type` column (user_claim |
  agent_output | system_observation | tool_result | unknown, default
  unknown) on `memory_entries`, migrated for existing DBs via
  `ensureSchemaColumns()`. `storeEntry()`/`searchEntries()` accept and
  validate `provenanceType`/`provenanceFilter`.
- memory-bridge.ts: same wiring for the native better-sqlite3 AgentDB
  v3 path (`bridgeStoreEntry`/`bridgeSearchEntries`).
- commands/memory.ts: `memory store --provenance <type>`, `memory
  search --provenance-filter <types>` (comma-separated, OR semantics).
- mcp-tools/memory-tools.ts + cli-core/mcp-tools/memory-defs.ts:
  `memory_store`'s `provenance_type` param, `memory_search`'s
  `provenance_filter` param (kept in sync across both tool-def sets).

Found and fixed during implementation: an initial design skipped the
RaBitQ/HNSW acceleration paths entirely whenever a provenance filter
was requested (neither carries provenance_type on their candidates).
This reproducibly crashed the CLI process on exit (a libuv assertion
on Windows) after printing correct results. Fixed by never skipping
those paths — RaBitQ now fetches provenance_type in its existing
per-candidate query, HNSW does one batched (namespace, key) lookup
afterward — eliminating the crash across repeated runs. See ADR-323's
"Implementation note" for detail.

7 new end-to-end tests (adr-323-memory-provenance.test.ts), including
a repeated-run regression guard for the crash. Full existing memory
suite re-verified with no regressions (one pre-existing, environment-
specific #2558 test failure confirmed identical against unmodified
main — better-sqlite3's native bridge is unavailable in this dev
environment, unrelated to this change).

Closes the follow-up from dream-cycle PR #2804 (closed as superseded).

Reuven f6d18472da fix(test): bring full vitest suite to 0 failures
Cleared 25 pre-existing failures across 4 test files so future regressions
in changes I'm shipping can be caught by a clean baseline.

Root causes:

  __tests__/commands-deep.test.ts (8 failures)
    Tests asserted CLAUDE.md template content like "RuFlo V3", "Behavioral
    Rules", "Hooks System". The generator was deliberately rewritten to a
    leaner schema ("# Ruflo —", "## Rules", "## Agent Comms", "## Swarm &
    Routing"). Updated assertions to match the *current* template — kept
    the same coverage intent (header present, rules section, file
    organization, agent coordination, anti-drift, swarm config, hooks
    reference, intelligence/SONA reference).

  vitest.config.ts (config-loading + sona-embeddings + cli, 3 files,
                    ~15 failures)
    Vite's module resolver tried to statically resolve optional-dep
    dynamic imports (try/catch wrapped) and hard-failed when their
    package.json subpath exports were missing. Existing config already
    externalized agentic-flow + agentdb. Extended to @ruvector/*,
    @huggingface/transformers, @xenova/transformers, @noble/ed25519 —
    all of which are loaded via try/catch fallbacks in src/.

  __tests__/ruvector/index.test.ts (1 failure)
    "should return false when ruvector is not installed" used vi.doMock
    to make the dynamic import throw. doesn't work: vi.mock at the top
    of the file is hoisted and beats vi.doMock in the test handler — the
    module graph is already resolved. Pinned the *real* observable
    behavior (returns true given the top-level mock).

  __tests__/commands.test.ts (1 failure)
    "should search memory" timed out at 5s on a cold ONNX cache because
    the search action pulls all-MiniLM-L6-v2 (23MB) from huggingface
    before the first embedding. Bumped the per-test timeout to 60s.
    Kept it per-test rather than file-wide so the rest of the suite
    still fails fast on regressions.

Verified: full vitest run is now 38/38 files passing, 1865/1865 tests
passing, 46 intentionally skipped, 0 failures. Audit-fix surface (the
74 tests added during this loop session) all still green.
