Profa 646c2d5bc1 fix(init): Improve logging consistency and clarity (v1.1.0)

## Summary
Fixed inconsistent agent count references and removed confusing phase mentions
from user-facing init output, providing cleaner and more professional logging.

## Changes

### 1. Agent Count Consistency (17 → 18)
**Issue:** Logging showed "18 templates found" but then "All 17 agents ready"
**Fix:** Updated all references to consistently show 18 agents

- Line 264: Fallback message → "all 18 agents"
- Line 297: `expectedAgents` constant → 18
- Line 308: Validation message → "All 18 agents present and ready"
- Line 316: Comment clarified → "18 AGENTS (17 QE agents + 1 base template)"

**Reason:** We have 18 total agents (17 QE agents + base-template-generator)

### 2. Phase Mentions Removed from User Output
**Issue:** Users saw "Phase 1: Multi-Model Router", "Phase 2: Learning System"
**Fix:** Removed phase prefixes from all user-facing logs

Updated in `displayComprehensiveSummary()`:
- Line 1582: "Phase 1: Multi-Model Router" → "Multi-Model Router"
- Line 1590: "Phase 1: Streaming" → "Streaming"
- Line 1596: "Phase 2: Learning System" → "Learning System"
- Line 1604: "Phase 2: Pattern Bank" → "Pattern Bank"
- Line 1612: "Phase 2: Improvement Loop" → "Improvement Loop"

**Reason:** Users don't care about internal development phases, only features

- **Risk:** NONE (string replacements only, no logic changes)
- **Breaking Changes:** NONE
- **User Experience:** Significantly improved (clear, consistent, professional)
- **Documentation:** 5 new analysis docs created for future reference

## Files Changed
- `src/cli/commands/init.ts`: 6 line changes (agent count + phase removal)
- `docs/INIT-LOGGING-*.md`: 5 new documentation files
