Records the 2026-06-17 evidence: ADR-038 (DRACO ceiling) is now Accepted
upstream → the A3 gate leans G-ABORT; MetaHarness v0.1.15 ships mcp-scan --json
+ scorecard, shrinking A1 to "consume, not build"; and two new standalone
candidate actions lifted from ADR-150 (A9 graceful-degradation, A10 KRR router).
Cross-links the ADR-150 spawnSync bug class to AQE #528.
