Profa 5022d762e9 fix(persistence): prevent DB corruption, remove VACUUM, fix dual-DB and stop hook (#260) ...

- Move seed flag set BEFORE cross-domain transfer to prevent re-runs on timeout
- Remove storeVector() call that caused 124MB vectors table bloat (30K+ rows)
- Fix findProjectRoot() to find topmost .agentic-qe dir (prevents split-brain)
- Fix hypergraph.db path to use findProjectRoot() instead of relative path
- Remove automated VACUUM from hybrid-backend (unsafe with concurrent hooks)
- Fix stop hook output format (remove invalid hookSpecificOutput)
- Clean up 52MB of v2 test temp files
