Profa 7ba1650ee6 feat: Phase 4 - TDD subagent coordination (1.8.3) ...

Implements comprehensive TDD subagent coordination solving the disconnected
tests/code/refactor issue where RED-GREEN-REFACTOR cycle agents were producing
inconsistent outputs.

## Added
- TDD Coordination Protocol with memory-based coordination using
  aqe/tdd/cycle-{cycleId}/* namespace
- File hash validation ensuring test file integrity across phases
- Handoff gates with readyForHandoff boolean
- 3 new subagents: qe-flaky-investigator, qe-coverage-gap-analyzer,
  qe-test-data-architect-sub
- TDDPhaseValidator class for runtime enforcement
- 22 integration tests for TDD coordination
- Coordination guide documentation

## Changed
- Updated 8 existing subagents with coordination protocol
- Updated example orchestrator with real MCP patterns
