Runs standalone or in GHA. - New workflow: .github/workflows/qcsd-production-trigger.yml Triggers after npm publish (workflow_run), weekly schedule, or manual dispatch. Collects telemetry, commits to repo, creates GitHub Issue with invocation command for the production swarm. - Edit: .claude/hooks/cross-phase-memory.yaml Added Loop 5 (CI/CD → Production deploy trigger) with post-release-decision and pre-production hooks.
