Lalit 669e1e1d9f refactor(qcsd): rename Grooming Swarm to Refinement Swarm across codebase ...

Rename the QCSD "Grooming" phase to "Refinement" in all TypeScript types,
YAML hooks, agent definitions, architecture docs, and catalogs. This aligns
with modern Scrum terminology (backlog refinement vs grooming).

- Update QCSDPhase type literal: 'grooming' → 'refinement'
- Rename hook identifiers: pre-grooming-* → pre-refinement-*
- Update cross-phase feedback loop names (Loops 2 & 4)
- Update agent cross_phase_memory sections
- Update MCP server phase enums
- Update architecture docs, catalogs, demos, and changelog
