fix: add root-level preinstall script for migration detection
Detects and provides clear instructions for users upgrading from:
- @agentic-qe/v3 (alpha) → agentic-qe@latest
- agentic-qe v2 → agentic-qe@latest
Supports AQE_AUTO_MIGRATE=true for automatic uninstall/reinstall.
Fixes binary conflict errors (EEXIST) when upgrading.
