Dragan Spiridonov e044ae7787 Fix learning persistence issue (#80) 
* fix(learning): persist learning data to SQLite database (Issue #79)

Root cause: Database schema was missing columns that handlers tried to write to:
- patterns table: missing domain, success_rate columns
- q_values table: missing metadata column
- learning_experiences table: missing metadata, created_at columns

Additionally, MemoryStoreHandler had persist:true flag but never actually
persisted to SQLite - it only wrote to in-memory Map.

Changes:
- SwarmMemoryManager.ts: Added missing columns to table schemas
- SwarmMemoryManager.ts: Added migrations for existing databases
- memory-store.ts: Now actually persists to SQLite when persist=true
- server.ts: Pass SwarmMemoryManager to MemoryStoreHandler

Verification:
- Created verify-issue-79-fix.ts script for testing
- All learning data (patterns, Q-values, experiences) now persists
- Data survives database reconnection

Fixes #79

* chore(release): prepare v1.9.2 release

- Update package.json version to 1.9.2
- Update README.md with v1.9.2 release info
- Update CHANGELOG.md with release notes for Issue #79 fix
- Remove deprecated/unused config files and commands (user cleanup)
- Learning persistence fix verified working
