Dragan Spiridonov a39d4ad010 ci(security): add consumer-side audit gate (npm overrides workaround)
Closes the gap surfaced by the @xenova/transformers → protobufjs
investigation: `npm audit` in this repo lies about user-facing CVEs
because the top-level `overrides` block in package.json forces patched
versions of vulnerable transitives — but npm `overrides` are root-only
by design and do NOT propagate to downstream consumers. A clean
`npm install agentic-qe` from a user's perspective can therefore see
high/critical CVEs that our in-repo audit reports as 0.

Two new gates run against an actual consumer install tree:

1. `consumer-audit` job in npm-publish.yml (BLOCKING pre-publish).
   Packs the local build with `npm pack`, installs the tarball into a
   fresh tmp project, runs `npm audit --audit-level=high` there. Now a
   `needs:` of the publish job, so a CVE invisible to repo-level audit
   still blocks publish.

2. Consumer-audit step + P1 issue creation in post-publish-canary.yml
   (NON-BLOCKING). Same idea, but against the freshly-published tarball
   from the registry — catches discrepancies between what we built and
   what the CDN serves. Findings open an auto-labelled P1 issue
   (security/canary/release) and surface in the run's step summary.

The two together replicate the actual user view at both the
pre-publish gate and the post-publish safety net.

Local simulation of the consumer-audit logic on the current head:
183 packages installed, 0 vulnerabilities — confirming the
@huggingface/transformers migration in 122f223c actually fixed the
consumer view (was 4 high + 1 critical pre-migration).

Dragan Spiridonov d8a04b22d2 ci(security): add npm audit gate on high/critical CVEs
Adds a new `security-audit` job to optimized-ci.yml that runs in parallel
with the journey shards and fails the build on any high/critical CVE in
production deps. devDep CVEs are reported as warnings — they don't ship
to users but are visible so we can fix them before they migrate.

Also adds the same `npm audit --audit-level=high --omit=dev` step to the
publish workflow's build job as a defense-in-depth gate: blocks `npm
publish` if a new prod-tree CVE has landed since the last main-branch CI
run.

Would have caught the 6 high `@typescript-eslint`/`minimatch` advisories
that Tier B just cleared the moment they were published, rather than
when someone happened to run `npm audit`.

Dragan Spiridonov 2f39f2210b ci(release): gate publish on daemon-runtime integration tests (#491)
Issue #491 exposed a structural gap in the release process: integration
coverage of the daemon-runtime seam existed (bridge-end-to-end test was
already in the repo), but it was never run on the release path —
`tests-on-tag-sha` only ran `test:unit:fast`, which excludes
`tests/integration/` by design. Every one of the four bugs in #491
lived in that unrun surface.

Two changes:

1. `package.json` — define `test:integration:fast` and
   `test:integration` scripts. The "fast" variant runs the focused
   daemon-runtime seam suite in ~6s:
     - tests/integration/mcp/fleet-init-wires-daemon.test.ts (Bug 1+4b)
     - tests/integration/workers/workers-reach-domains.test.ts (Bug 1 generalized)
     - tests/integration/learning/coordinator-roundtrip.test.ts (Bug 2+3)
     - tests/integration/bridge/bridge-end-to-end.test.ts (existing)
   The broader `test:integration` runs all of `tests/integration/`
   excluding browser/e2e for local development use.

2. `.github/workflows/npm-publish.yml` — add a new
   `integration-tests-on-tag-sha` job that runs `test:integration:fast`,
   wired into the publish job's `needs` and `if` conditions in the same
   shape as the existing `tests-on-tag-sha`. Picked the "fast" variant
   deliberately: it covers exactly the regressions #491 reported and
   keeps the release gate budget tight (~10-min job ceiling, but the
   work itself is ~6s). The broader integration suite is too noisy for
   a release gate; the fast subset is the floor.

This is the structural complement to issue #401's pre-publish init
gate: that one catches `aqe init` regressions before users, this one
catches daemon-runtime regressions before users.

Dragan Spiridonov 2d35843ce8 fix(ci): wrap Vibium's Chrome to force --no-sandbox on GitHub runners
Diagnostic output from commit a4cb96c5 confirmed the residual error
is chromedriver's classic "HTTP 500 create session" — Chrome's
setuid sandbox can't initialize in the GitHub Actions runner
environment (no user namespaces, restricted proc).

Fix: after `vibium install` lays down Chrome for Testing into
~/.cache/vibium/chrome-for-testing/<version>/chrome, replace the
binary with a tiny bash wrapper that execs the real Chrome with
--no-sandbox --disable-dev-shm-usage appended. The wrapper lives
inside the ephemeral runner cache; nothing is shipped to users.

This is the standard workaround used by every Chromium-based CI
runner (Puppeteer, Playwright, chromedp all do the equivalent).

Also removed the diagnostic step added in a4cb96c5 (job green is
the signal now).

Dragan Spiridonov a4cb96c524 diag(ci): probe residual Vibium failure post-install (not a fix) 2026-04-22 11:38:41 +00:00
Dragan Spiridonov 8a90c10f18 fix(ci): run vibium install to download Chrome for Testing + chromedriver
Root cause confirmed via diagnostic (commit 0f810e3f): Vibium's
auto-launch fails with "failed to create session: HTTP 500"
because the npm package ships only the Go binary — the actual
browser and chromedriver are downloaded on demand by
`vibium install`. The earlier VIBIUM_BROWSER_PATH fix was a red
herring: Vibium drives its own bundled Chrome for Testing over
WebDriver, not the runner's system google-chrome.

Reverted:
- "Discover browser path for Vibium" step (both jobs)
- "DIAGNOSTIC — probe Vibium failure" step (smoke job)

Added:
- "Download Chrome for Testing + chromedriver" step running
  `vibium install` after the npm install, in both smoke and eval
  jobs. This matches the documented setup flow for Vibium.

Dragan Spiridonov 0f810e3f71 diag(ci): probe Vibium smoke failure — not a fix
TEMPORARY diagnostic step to surface the real error Vibium's
wrapper is hiding behind "stdout: null, stderr: null". Runs a
battery of probes with continue-on-error so the step never blocks
the smoke job itself, and prints enough to diagnose whether the
Go binary is crashing on sandbox, /dev/shm, missing lib, or
something else.

To be reverted in the next commit once the root cause is known.

Dragan Spiridonov 956fd26ef1 fix(ci): discover browser path for Vibium on GitHub runners
Root cause of PR #431 smoke/eval job failures: Vibium installs
correctly (reports v26.3.18) but exits 1 when it tries to drive a
browser, because the runner has no Chrome on its default PATH for
Vibium's discovery — smoke-test tc002/tc004/tc006-8/tc010 all crash
with `vibium eval exited 1` in child_process, while tests that
don't need a real navigation (tc003, tc005, tc011) pass.

Fix: added a preflight step in both `smoke` and `eval` jobs that
probes the same prioritized list the installer uses
(google-chrome → google-chrome-stable → chromium-browser → chromium),
exports VIBIUM_BROWSER_PATH via $GITHUB_ENV, and fails loudly if
none is found. Ubuntu-24.04 runners have google-chrome preinstalled,
so this resolves without adding any third-party action.

This mirrors the remediation that `aqe init --auto` prints on Linux
ARM64 hosts — the mechanism is identical, just applied on AMD64 CI
where Vibium's auto-detection also fails.

Validated with actionlint 1.7.1 (clean).

Dragan Spiridonov fe568cfe45 fix(ci): set least-privilege permissions on qe-browser workflow
Resolves 3 CodeQL findings (205/206/207) from github-advanced-security
on PR #431: "Workflow does not contain permissions".

Added workflow-level `permissions: contents: read`. The workflow only
reads the repo and uploads an artifact — no writes to code, issues,
PRs, or releases. The artifact upload uses the ephemeral job token,
not the `actions: write` permission.

Validated with actionlint 1.7.1 (clean).

Dragan Spiridonov cda9fb3c05 fixup(adr-091): address devil's-advocate review
Single commit covering the 5 concrete findings from the qe-devils-advocate
review. Does not change the underlying CommandEvalRunner design or the 5
primitives — only the claims around them and the fixture-server hack the
yaml had silently inherited.

1. Revert promotion (G4): trust_tier 3 -> 2 in SKILL.md; ADR-091 status
   Implemented -> In Progress (2026-04-21); v3-adrs.md index row updated
   to reflect the same; counts 80->79 Implemented, 6->7 In Progress.
   The promotion gate is now documented on ADR-091: first green CI run
   of test-qe-browser.yml (unit + smoke + eval against real Vibium)
   before trust_tier 3 / Implemented are restored.

2. Fix yaml fixtures (G3): tc006/tc007 were referencing
   http://localhost:8088/qe-browser/SKILL.md.html which required an
   unstarted fixture server, so `aqe eval run --skill qe-browser` would
   have failed on a fresh machine. Rewrote both to use https://httpbin.org/html
   (matching scripts/smoke-test.sh tc006/tc007). Dropped tc009
   (poisoned-page check-injection) from the yaml — it needs the local
   serve-skills.js fixture which is out of scope for CommandEvalRunner.
   Documented as an explicit GAP comment in the yaml, noted that
   check-injection.js's severity path is unit-tested at
   tests/unit/scripts/qe-browser-check-injection.test.ts. yaml test_cases
   now 11 -> 10.

3. Correct pin comment wording (G1): the previous comment claimed
   ^26.3.18 was a "major.minor-line pin" — that's factually wrong under
   npm semver (^26.3.18 accepts 26.99.x). Rewritten to accurately
   describe the intent: major-line pin that blocks 27.0+ while
   allowing auto-uptake of 26.x security patches and additive features,
   with scripts/smoke-test.sh as the belt-and-suspenders API contract
   gate. The spec itself stays ^26.3.18 — the correct choice for an
   end-user tool under semver assumptions.

4. Add eval CI job (G2): third job in test-qe-browser.yml runs
   `aqe eval run --skill qe-browser --model claude-3.5-sonnet` against
   a real Vibium install after `npm run build`. This is the
   end-to-end proof that CommandEvalRunner dispatch works that the
   previous unit tests (all mocked CommandRunner) could not provide.
   Uploads the eval JSON as a CI artifact for post-run inspection.
   actionlint clean.

5. Scrub stray files (G5): git rm the coherence-comparison artifacts
   from docs/reports/ that were swept in by a prior `git add -A`, and
   added a focused pattern to .gitignore (not a blanket docs/reports/
   ignore) so future sessions can't repeat the mistake while intentional
   reports can still be committed.

Tests: 51 relevant tests still pass (3 files: command-eval-runner,
command-eval-runner-qe-browser, browser-engine-installer).

Dragan Spiridonov 4739237cd8 feat(adr-091): migrations, getting-started doc, CI workflow
Closes GAP-03a, GAP-03b, GAP-06, GAP-07, and revises GAP-09.

- Rephrased Playwright references in enterprise-integration-testing and
  pentest-validation to point at qe-browser (batch/assert/check-injection)
  so the AQE fleet's canonical browser runner is the recommended tool.
- accessibility-testing: added qe-browser / a11y-ally as the preferred
  axe-core integration path ahead of the Playwright example; Playwright
  kept as a documented fallback for Firefox/Safari or existing suites.
- GAP-09 revised: visual-testing-advanced and compatibility-testing are
  intentional hybrids, not unfinished migrations. visual-testing-advanced
  already labels its Playwright section "LEGACY: fallback"; compatibility
  -testing notes that Vibium's BiDi backend is Chrome-only at v26.3.x so
  Firefox/Safari coverage still needs Playwright. Both stay as-is.
- Added docs/guides/qe-browser-getting-started.md covering install,
  smoke check, eval run, and the 5 primitives with example CLI calls.
- Added .github/workflows/test-qe-browser.yml gating PRs that touch the
  skill, its installer, or CommandEvalRunner. Job 1 runs the full vitest
  sweep; Job 2 installs a pinned Vibium and runs smoke-test.sh against
  real httpbin fixtures.
- Workflow validated with actionlint 1.7.1 (clean).

Dragan Spiridonov 9d3b514ee2 fix(v3.9.14): second P0/P1 wave from QE audit follow-up
Security (M-01):
- src/cli/commands/learning.ts: replace execSync shell-interpolation of
  the user-controlled --file path with execFileSync + file-descriptor
  redirect (dump) and stdin input (import). Closes the command-injection
  vector where a crafted --file="x.db\"; rm -rf ~; \"" would escape
  shell quoting.

Build / supply chain:
- Move @claude-flow/guidance from dependencies to optionalDependencies
  alongside @claude-flow/browser. No stable successor exists on npm;
  usage is already guarded by try/await-import/catch fallbacks so
  demotion is safe. Documented in the follow-up research memo.
- Lazy-load @faker-js/faker via dynamic import in
  test-data-generator.ts and mark it external in build-cli.mjs /
  build-mcp.mjs. Faker is a devDep that previously got statically
  bundled into shipped artefacts because of a top-level import. Tarball
  drops 11.5 MB → 9.6 MB.

MCP API contracts:
- Add required:true to semantically load-bearing first parameters on
  agent_metrics (agentId), coverage_analyze_sublinear (target),
  accessibility_test (url), defect_predict (target), code_index
  (target). Verified via live MCP tools/list handshake — required
  arrays now match the intent documented in each tool's description.

CI hygiene:
- Rewrite the "Commit telemetry data" step in
  qcsd-production-trigger.yml to push to a bot branch and open a PR
  instead of `git push` direct to protected main. That push failed 8/10
  runs on the last audit cycle because of branch protection. Artifact
  upload (already present) continues to preserve the raw payload for 90
  days regardless of whether the PR lands.

Verification:
- npm audit --omit=dev: 0 critical, 0 high (1 moderate in
  @mapbox/node-pre-gyp — extraneous, pre-existing, unrelated).
- npm run typecheck: clean.
- CLI + MCP bundles rebuild successfully.
- 253/253 test-generation unit tests pass.
- Live MCP protocol smoke: new required flags visible on tools/list;
  advisor_consult guard from v3.9.14 round 1 still fires.

Memory note: see feedback_cli_is_aqe.md — CLAUDE.md ruflo refs were
audited. Only 2 lines (204, 260-261) are stale AQE-product refs; the
other 11 swarm/memory/daemon ruflo commands are legitimate
orchestration-layer invocations and are intentionally retained.
Deferred to a dedicated docs PR so the user can review each.

Dragan Spiridonov 4a641e7ef1 fix(adr-093): post-audit sweep — CI workflow, consensus provider, MCP sampling, 12 skill evals, 9 test files
Fifth post-audit sweep. 24 files, +66/-60.

CRITICAL (user-facing):
* .github/workflows/skill-validation.yml:422: CI eval runner
  --model claude-sonnet-4 → claude-sonnet-4-6. Previously CI would
  target a retiring model.
* src/coordination/consensus/providers/claude-provider.ts: Claude 3.x
  consensus provider modernized — type union extended with
  claude-sonnet-4-6 / claude-opus-4-7 / claude-haiku-4-5 as first
  entries, default model updated from claude-3-5-sonnet-20241022
  to claude-sonnet-4-6, availableModels list prepends new models.
  Legacy 3.x entries retained for backward compat.
* src/mcp/security/sampling-server.ts: default model
  claude-3-sonnet-20240229 (retired 2026-02-19) → claude-sonnet-4-6.

Skill evals (12 files, 6 + 6 mirrors):
* .claude/skills/ and assets/skills/ mirrors for:
  performance-testing, chaos-engineering-resilience, quality-metrics,
  localization-testing, accessibility-testing,
  n8n-trigger-testing-strategies. models_to_test list updated to
  claude-sonnet-4-6 / claude-haiku-4-5.

Test files (9):
* tests/unit/coordination/consensus/providers/claude-provider.test.ts:
  default model, test descriptions, and references updated to 4.6/4.7.
* tests/unit/routing/economic-routing.test.ts: claude-3-opus-20240229
  usage → claude-opus-4-7.
* tests/unit/learning/skill-validation-learner.test.ts: claude-3-haiku
  / claude-3.5-sonnet / claude-3-opus → new canonicals.
* tests/unit/mcp/handlers/handler-test-utils.ts: claude-3-haiku →
  claude-haiku-4-5.
* tests/integration/llm/{real-provider-integration,multi-provider}.test.ts:
  bare 'claude-sonnet-4' canonical → 'claude-sonnet-4-6'.
* tests/integration/validation/parallel-eval-runner.test.ts:
  claude-3.5-sonnet / claude-3-haiku → new canonicals.
* tests/routing/advisor/multi-model-executor.test.ts:
  CRITICAL BUG — 'claude-opus-4-6' (non-existent model ID, typo) ×2
  → 'claude-opus-4-7'. Stale test name 'anthropic/claude-opus-4
  when no provider' → '-4.7'.
* tests/unit/opencode/degradation.test.ts: CRITICAL BUG —
  modelId 'claude-opus-4-6' (typo) → 'claude-opus-4-7'.

Left intentionally untouched:
* tests/unit/coordination/consensus/providers/openrouter-provider.test.ts:
  tests that OpenRouter provider lists claude-3-haiku / claude-3.5-sonnet
  / claude-3-opus as supported. These assertions reflect actual
  OpenRouter provider data, which hasn't changed. Adding new-model
  assertions would be an enhancement, not a fix.

Verification:
* 0 tsc errors
* 174/174 tests across touched paths pass

Dragan Spiridonov 4f340bc183 feat(verification): chaos workflow + VERIFICATION.md + matrix embed (#409, #410)
Closes the two remaining init-corpus follow-ups from #401 in one PR:
makes the verification layer visible to humans (#409) and adds
adversarial-rare coverage to complement the everyday-real release
gate (#410).

# #410 — Chaos workflow

Weekly cron-driven workflow that runs aqe init against 6 pathological
project shapes the everyday-real corpus deliberately doesn't cover.
Per-shape assertion is "exit anything except 124" — the watchdog is
the load-bearing thing under test, not init's ability to make sense
of garbage input. Fail-loudly in the Actions tab; no auto-issue
creation, matching the #411 mirror-test pattern.

Six shapes: utf16le-bom, mixed-line-endings, symlink-loop,
binary-as-text, minified-bundle, control-chars. Generated at runtime
into a tmpdir by tests/fixtures/init-chaos/generate.sh — synthetic
generation is appropriate here per the chaos README's "Why synthetic
generation here" section: chaos tests need controlled adversarial
inputs that don't exist in any real public repo.

Verified locally: ran the generator + ran aqe init against all 6
shapes. All 6 pass with rc=0 against the current build, proving the
generator produces valid shapes and the existing watchdog handles
them today. The workflow exists to catch the day a future change
breaks one of these paths.

# #409 — VERIFICATION.md + matrix embed

New docs/VERIFICATION.md is the maintainer-facing entry point: gate
architecture diagram, how to interpret a failed gate, how to add a
new fixture (links to corpus README), how to embed the verification
matrix into a release notes file.

scripts/embed-verification-matrix.sh downloads the init-corpus-logs
artifact from a given workflow run, parses summary.txt, and emits a
markdown matrix. Status-only (option C from the proposal) — Time and
KG-entries columns are tracked as future work because the gate's
current summary.txt doesn't carry that data and extending it would
touch load-bearing infrastructure that's out of scope.

docs/policies/release-verification.md is fully rewritten around the
post-#401 reality: the manual aqe init checklist that pre-dated the
corpus is removed (the gate now does what it tried to do, with
22 assertions per fixture × 4 fixtures × the load-bearing doubleInit
flag). The Version Update Policy + Workflow sections are preserved
intact — they cover the one class of pre-release error the gate
cannot catch.

CONTRIBUTING.md links to the new VERIFICATION.md from the
Documentation section.

# Housekeeping

Removes 3 stale rows from tests/fixtures/init-corpus/README.md's
deferred-items table: #408 (already merged), #409 and #410 (this
PR). Only #407 remains — its 4 trigger conditions still haven't
fired, so it stays deferred per its own definition.

# Verification of this PR (per #401 rules)

- actionlint passed on .github/workflows/init-chaos.yml (#408 lesson:
  js-yaml is not enough for workflow files)
- bash -n on both new shell scripts
- generator ran cleanly and produced all 6 shapes with the expected
  byte content (verified BOM, line endings, symlink targets, PNG
  signature, single-line bundle size, embedded NULs)
- embed script ran against synthetic summary files (PASS-only and
  mixed PASS/FAIL) — both render correctly
- live smoke test: ran aqe init against all 6 chaos shapes locally
  using the globally-installed aqe binary, all 6 exited rc=0 within
  the watchdog budget
- every link in docs/VERIFICATION.md and docs/policies/release-
  verification.md verified to point at a file that exists

Dragan Spiridonov 4ba21d365d fix(pr-template-check): escape \${{ }} template in shell comment
The first workflow run failed to parse at all — GitHub's workflow
parser scans the entire run: block for \${{ ... }} regardless of shell
comment syntax, so the text "${{ }}" inside a shell comment was read
as an empty expression and rejected with "unexpected end of input".

Moved the explanatory note from a shell comment (inside the run:
block) to a YAML comment (outside the run: block) so the workflow
parser never sees the literal \${{ pattern.

Caught by actionlint — js-yaml validated the file but actionlint
knows about GitHub's expression scanner and flagged the empty
expression with the right error message.

Dragan Spiridonov 70713b0b45 feat(pr-template-check): CI enforcement for the #401 failure-modes checkbox
Replaces the honor-system PR template checkbox with a parser that fails
any PR whose body either (a) unchecks or deletes the load-bearing
failure-modes checkbox, or (b) contains dismissal phrases like
"I believe it's unlikely" without a same-paragraph tracking issue
reference. Both rules hard-fail.

Covers the 3 phrases explicitly named in #408 plus 8 additional
dismissal patterns that shipped or could ship the same class of
reasoning that caused the v3.9.1–v3.9.4 init regression series.

- scripts/check-pr-body.cjs: standalone Node parser, ESM-safe via .cjs,
  zero dependencies, exported for self-test. Skips the template's own
  checkbox paragraph when scanning for forbidden phrases — the stock
  instruction text quotes the phrase as a teaching example and would
  otherwise trip the linter on every clean PR.
- .github/workflows/pr-template-check.yml: two jobs. lint-pr-body runs
  the parser against the actual PR body (dependabot[bot] allowlisted).
  self-test runs the parser against 11 fixtures and asserts both
  exit code and a content-specific substring in the output, so a
  parser regression fails loudly instead of silently passing bad PRs.
- tests/fixtures/pr-body/: 3 valid fixtures (clean, tracked-phrase,
  tracked-edge-case), 6 invalid fixtures (unchecked, missing section,
  forbidden phrase, multi-violation, highly-unlikely, can't-see-how),
  2 historical fixtures (the actual v3.9.3 PR body which fails for
  missing section, plus a synthetic body quoting #401's postmortem
  reference which fails for forbidden phrase).

Rollout: workflow is intentionally not required at merge time on
initial landing. Maintainer should promote it to a required status
check in branch protection once it has been observed to behave
correctly on live PRs — see the header comment in
.github/workflows/pr-template-check.yml.

Verified locally with the same self-test logic the workflow will run:
11/11 fixtures pass with content-aware assertions, and a mutation
test (breaking the checkbox-state logic) correctly causes the valid
fixture to fail, proving the self-test would catch a parser regression.

Dragan Spiridonov 990230d141 fix(init-corpus-mirror-test): set least-privilege GITHUB_TOKEN permissions
CodeQL flagged the new workflow for an unrestricted GITHUB_TOKEN. This
workflow only needs to checkout the repo and run scripts — no writes,
no PR comments, no issue creation — so contents:read is sufficient.
Matches the pattern already used by npm-publish.yml, coherence.yml,

Dragan Spiridonov 7f46ff88f1 feat(init-corpus): mirror tarballs to GitHub Releases for codeload drift resilience
Populates the tarball.mirror field reserved in MANIFEST v2 and wires
setup.sh to fall back to the mirror when codeload.github.com returns
either a network error or bytes whose sha256 no longer matches the
manifest — the exact failure mode that has hit GitHub's git archive
toolchain at least twice (2023, 2024). Without this, the release gate
would hard-fail on the next codeload regeneration until a maintainer
manually rewrote the pinned checksums.

- scripts/upload-init-corpus-mirror.sh: idempotent maintainer tool that
  downloads each fixture from primary, verifies sha256 against MANIFEST,
  and uploads as <sha256>.tar.gz to the init-corpus-v1 release. Refuses
  to upload on sha mismatch so drifted bytes can never reach the mirror.
- tests/fixtures/init-corpus/setup.sh: primary→mirror fallback via a new
  download_and_verify helper. Emits loud WARNING lines on fallback so
  CI surfaces codeload drift even when the job stays green. Adds
  AQE_CORPUS_MANIFEST_PATH and AQE_CORPUS_FORCE_MIRROR env overrides for
  the mirror test workflow.
- .github/workflows/init-corpus-mirror-test.yml: exercises both the
  mirror-reachability path (FORCE_MIRROR=1) and the fallback-on-drift
  path (local python3 http.server serves bogus primary bytes). Runs on
  every PR that touches the corpus and weekly on Mondays.
- MANIFEST.json: populates tarball.mirror for tiny-ts, mid-ts, and
  multi-lang-real with the now-uploaded release assets.

Dragan Spiridonov accb9a774d fix(release-gate): share single npm install across fixtures
The v3.9.7 publish attempt hit the `pre-publish-gate` job's 10-min
timeout because `run-gate.sh` was doing a fresh `npm install
agentic-qe-X.Y.Z.tgz` in each of the 4 fixture cleanrooms. On CI
runners each install takes 2-3 min (better-sqlite3 + hnswlib-node
prebuilds + the full transitive tree). Total install budget ~10-12 min,
blowing the timeout even though `aqe init` itself is 0-5s per fixture.

Fix: install agentic-qe ONCE into a host dir at the top of run-gate.sh
and invoke the binary via absolute path from each fixture cleanroom.
This is what a real user does with `npm install -g agentic-qe` + run
init in any project dir.

Changes:
  - run-gate.sh: new HOST_DIR one-time install, AQE_BINARY used by
    all fixtures instead of cleanroom-local node_modules/.bin/aqe.
    A5/A6 assertions removed from the comment block (they're now
    enforced once at the top of the script, not per-fixture).
  - run-gate.sh: kg_entries query (A12) now uses NODE_PATH pointing
    at HOST_DIR/node_modules so better-sqlite3 resolves. Same for
    the workers registry query (A19).
  - run-gate.sh: cleanup host dir at exit unless AQE_GATE_KEEP_HOST=1
    or AQE_GATE_HOST_DIR is set externally.
  - run-gate.sh: A12 diagnostic now includes the error message (not
    just '-1') so future DB query failures are self-diagnosing.
  - npm-publish.yml: pre-publish-gate timeout-minutes bumped from 10
    to 20 as a safety net. Shared install takes 143s locally; CI
    runners are slower but well within 20 min.

Evidence:
  - Local full corpus run with shared install: 4/4 pass, host install
    143s, per-fixture init 0-2s each, total wall clock ~3 min.
  - All 22 assertions per fixture still pass (A5/A6 subsumed into
    the one-time host install check).

Dragan Spiridonov 05dcfe92dd fix(init): release-gate corpus + --json contract (#401)
Addresses the v3.9.1–v3.9.4 init regression series by building the
verification layer that #401's post-mortem identified as structurally
missing. The load-bearing changes are the real-public-repo corpus, the
--json contract, and the restructured error handling in phase 06 that
stops lying about success when a non-critical phase fails.

What landed
-----------
* `aqe init --json` — structured InitResult output with schema version 1,
  stricter exit code semantics (exit 1 on any step.status='error', not
  just on critical failures). Stable contract for the gate and other CI
  consumers. See InitJsonOutput in src/cli/handlers/init-handler.ts.

* Phase 06 no longer swallows errors as `{status:'skipped',entries:0}`.
  Exceptions now propagate to BasePhase.execute() and surface as real
  step errors in result.steps[]. Fixed a resource leak where kgService
  and memory handles were leaked on the old throw path — now always
  released via try/finally.

* `tests/fixtures/init-corpus/` — 4 pinned real public repo fixtures:
  - tiny-ts (developit/mitt@b240473)
  - mid-ts (sindresorhus/p-queue@55d306b)
  - multi-lang-real (ruvnet/RuView@2a05378 — contains the exact 28,745
    byte / 776 line ruview_live.py that triggered the v3.9.4 hang,
    sha256-pinned to detect content drift)
  - self-dogfood (agentic-qe HEAD)

* 22 gate assertions per fixture (A1–A22) including per-step JSON error
  inspection (A10), snapshot-based KG threshold with tolerance (A12),
  skills/agents/MCP/CLAUDE.md/workers/config.yaml presence (A13–A20),
  subthreshold stall detection (A21), and a second init pass that
  exercises the delta-scan path of phase 06 — the actual surface that
  hung in v3.9.1 ruview, never reached on a first init (A22).

* `.github/workflows/npm-publish.yml` — new pre-publish-gate job that
  runs the corpus against the freshly-built tarball before publish; new
  tests-on-tag-sha job that re-runs fast unit tests on release events
  (replaces a buggy gh-api conditional caught by devils-advocate review).

* `.github/workflows/post-publish-canary.yml` — fires on real publishes
  only (not workflow_dispatch dry runs), polls npm CDN via
  `npm install --dry-run` (actual CDN edge, not just registry metadata),
  runs the corpus against the published package, opens P0 issue on
  failure via heredoc-based body.

* Watchdog effectiveness unit test
  (tests/unit/init/phases/code-intelligence-watchdog.test.ts) — 4 cases
  proving the async-stall branches of runBoundedScan work and
  documenting the sync-block limitation that requires AQE_SKIP_CODE_INDEX
  or (future) worker isolation.

* `.github/PULL_REQUEST_TEMPLATE.md` — single load-bearing checkbox:
  every PR failure mode must have a test or tracking issue. Honor-system
  today; CI enforcement tracked in #408.

* AQE_SKIP_CODE_INDEX promoted from "temporary escape hatch" to
  permanent supported flag in docs, CLI help, and source comments.

Verification
------------
* `npm run typecheck` — clean
* `npx eslint` on all touched files — clean (plus fixed two pre-existing
  unused-var warnings in init-handler.ts)
* `aqe security --sast` on all touched source + workflows — 0 findings
* `tests/unit/init/` — 377/377 pass
* `tests/unit/init/phases/code-intelligence-watchdog.test.ts` — 4/4 pass
* Local gate against `npm pack`'d tarball — 4/4 fixtures pass all 22
  assertions including double-init delta path and sha256 regression
  marker

Failure paths independently verified (F1–F5 injection tests):
* F1 wrong sha256 → A3 fires, exit 2, actionable diagnostic
* F2 absurd expectedKgEntries → A12 fires, exit 2, shows actual vs expected
* F3 expectedElapsedSec=0 → A21 fires on slow fixture, A22 fires on
  double-init — same `(( elapsed > soft_cap ))` math proven in both paths
* F4 injected throw in runCodeIntelligenceScan → reproduces the exact
  v3.9.3 failure shape (`success:true` at orchestrator level, phase 06
  step.status='error'). A7 catches via strict exit code, A10 catches as
  defense-in-depth via jq inspection. Both layers verified.
* F5 expectedSkillsInstalled=999 → A13 fires with OR-logic showing
  json=84, disk=84 vs expected 999

Deferred with tracking issues
-----------------------------
* Phase 06 worker_threads isolation — #407
* PR template CI enforcement — #408
* VERIFICATION.md + release-notes matrix — #409
* Chaos workflow for pathological inputs — #410
* Mirror init-corpus tarballs to GitHub Releases — #411

What is NOT verified
--------------------
* The CI workflows have never actually executed in a real Actions run.
  YAML syntactically validated via js-yaml, job dependency graph traced,
  but the first real execution is the first push of this branch.
* The canary on a real publish is unexecuted. First execution is the
  first real release after merge.

* feat: implement ADR-062 (StrongDM all 3 tiers) and ADR-087 (RuVector 14/14 capabilities)

ADR-062 StrongDM Software Factory — all 6 actions across 3 tiers:
- Tier 1: Loop detection wired to MCP protocol-server (blocks on strike 3+),
  token dashboard (pre-existing)
- Tier 2: Holdout testing (10% FNV-1a selection, filtered in test-execution),
  gate ratcheting (monotonic 5-pass/2%/95% cap, persisted to MemoryBackend)
- Tier 3: Progressive context (TF-IDF prediction, reduces availableFiles on spawn),
  meta-learning cycle (4 insight types, stored + published as domain events)

ADR-087 RuVector Advanced Capabilities — milestones 1-5:
- EWC++ recordOutcome + backgroundConsolidate wired to 3 domain coordinators
- R1: HDC fingerprinting (10K-bit hypervectors, wired to PatternStore.store())
- R3: Delta event sourcing (sparse encoding, wired to PatternStore.store())
- R13: Cognitive routing (predictive compression, wired to mailbox.send/receive)
- R14: Hyperbolic HNSW (Poincare ball, auto-indexed in PatternStore.store())

Also updates ADR statuses: 059, 060, 061 → Implemented (were already done),
062 → Implemented, 086 → In Progress, 087 → Implemented.

223 new tests across 11 test files, all passing. Zero regressions.

* feat: implement RVF cluster (ADR-065/066/067/071) with native benchmarks

Implements the RVF persistence cluster for persistent HNSW vector storage,
agent memory branching, and HNSW unification foundation.

Key changes:
- RvfPatternStore: IPatternStore backed by @ruvector/rvf-node native HNSW
  (1.2ms cold-start, 0.19ms search p50, verified with real native binary)
- AgentMemoryBranch: per-agent COW isolation via derive(), ingest-log merge,
  wired into Kernel.initialize() + DefaultAgentCoordinator
- HnswLegacyBridge: adapts old IHNSWIndex to unified IHnswIndexProvider,
  wired into PatternStore when useUnifiedHnsw flag enabled
- Factory routing: createPatternStore() returns RvfPatternStore when flag
  enabled + native available + data directory exists
- Dream engine: accepts external RVF adapter (no duplicate file handles),
  RVCOWBranchManager enhanced with recall validation check
- @ruvector/gnn bumped 0.1.19→0.1.25, 3 new feature flags added
- IPatternStore interface extracted for polymorphic pattern store usage
- 50 new tests (16 RvfPatternStore + 17 AgentMemoryBranch + 12 bridge + 5 benchmarks)
- 1188 tests pass, 0 regressions

ADR status: 066 Implemented, 067 Implemented, 065 Implemented,
069/071 In Progress (honest assessment after brutal-honesty review)

* fix(init): resolve skills/agents paths from package root, not relative to bundle depth

esbuild code-splitting puts installer code into dist/cli/chunks/ (3 levels
deep) instead of dist/init/ (2 levels deep), so the old ../../ traversals
landed at dist/ instead of the package root — causing "Source directory not
found" errors for all global installs.

Replace brittle relative-path resolution in all 4 installers with a shared
findPackageRoot() that walks up to the nearest package.json with
name=agentic-qe. Prioritise assets/ paths (canonical npm source) over
.claude/ paths.

* feat: complete RVF cluster Phases 2-3 (ADR-067/069/071/072) with devil's advocate fixes

Phase 2A (ADR-067): Agent memory branching enabled — useAgentMemoryBranching=true,
BRANCH_DISCARD witness action, recordIngest callback exposed to agents via config.

Phase 2B (ADR-069): Dream RVF COW — useRvfFork driven by isRVFPatternStoreEnabled(),
child adapter stored + temp files cleaned up on merge/discard, 13 tests.

Phase 2C (ADR-071): HNSW unification — useUnifiedHnsw=true, HNSWEmbeddingIndex routes
through HnswAdapter, UnifiedMemory vectorIndex uses UnifiedHnswIndex, HnswLegacyBridge
delegates to adapter string-ID APIs (no duplicate ID maps), addEmbeddingsBatch routed
correctly, saveIndex/loadIndex safe under unified mode.

Phase 3 (ADR-072): RVF primary migration scaffolding — RvfMigrationAdapter (5-stage
routing with write latency tracking), RvfConsistencyValidator (rolling divergence +
compaction), RvfStageGate (go/no-go with witness chain), rvfMigrationStage=1 default,
wired into shared-rvf-dual-writer at stage >= 2, 32 tests.

Cross-cutting fixes from devil's advocate review:
- Shared RVF adapter singleton eliminates dual file handles (kernel + dream engine)
- RvfPatternStore reports honest nativeAvailable=false + rvfInitError on failure
- ESM-safe dynamic import() replaces require() in dream-engine and branch-manager
- quality_assess defaults to src/ instead of cwd to prevent full-project scan hang
- discoverSourceFiles skips .agentic-qe/.claude/.cache dirs, adds maxFiles=5000 cap

Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>

* feat: complete RVF cluster plan — Phase 2C decommission, Phase 3 integration, fix 7 pre-existing test failures

Phase 2C (ADR-071) — HNSW decommission:
- Remove RuvectorFlatIndex fallback from unified-memory.ts; always use UnifiedHnswIndex
- Hardcode isUnifiedHnswActive()=true in HNSWEmbeddingIndex (hnswlib-node legacy path is dead code)
- Create HnswShadowValidator for brute-force vs HnswAdapter divergence validation
- Benchmark alias (rvf-pattern-store.bench.ts) for plan compliance

Phase 3 (ADR-072) — RVF Primary Migration integration:
- RvfMigrationCoordinator: wires adapter + validator + gate with real SQLite/RVF handles
- MCP tools: migration_status, migration_check, migration_promote (persistence category)
- Kernel boot wiring: coordinator initializes when rvfMigrationStage >= 2
- RvfMigrationAdapter.setStage(): propagate promotions to routing logic
- Promote default rvfMigrationStage from 1 (hybrid) to 2 (dual-write-sqlite-primary)
- Integration test: 13 tests covering full lifecycle (stage 1→2→3→4), write/read routing,
  consistency checks, gate enforcement, fallback, metrics

Test fixes (7 pre-existing failures → 1 remaining):
- code-intelligence-metrics-wiring: spy console.info not console.log (logger routes to console.info)
- mcp-integration: disable ADR-062 loop detection (repeated identical calls are by design)
- dream-cycle-tool (2 tests): RvfPatternStore empty-query branch returns all patterns from SQLite
- coherence-integration (4 tests): RvfPatternStore vector search fallback computes brute-force
  cosine over SQLite embeddings when RVF adapter unavailable; getStats() falls back to SQLite count;
  auto-attach SQLitePatternStore in initialize() when not explicitly wired

* chore(release): bump version to v3.9.1

- Version bump in package.json and skills-manifest.json
- Fix "Dynamic require of X is not supported" in ESM bundles by adding
  createRequire banner to both CLI and MCP build scripts — every chunk
  now has a working require() for Node.js builtins
- Add CHANGELOG.md entry and release notes for v3.9.1

* fix(ci): regenerate package-lock.json for npm ci compatibility

The lock file was out of sync with package.json — new transitive
dependencies added on main were missing. All 15 CI checks failed
at `npm ci` with "Missing from lock file" errors.

Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>

* fix(ci): pin Node to 24.13.0 and restore working lock file

Node 24.14.1 ships npm 11.11.0 which enforces stricter lock file
validation than 11.6.x (what 24.13.0 ships). The existing lock file
passes under 11.6.x but fails under 11.11.0 with "Missing from lock
file" for transitive dependencies of private-registry packages.

Pin all 5 workflow files to node-version: '24.13.0' and restore the
lock file from commit f9b76968 (last CI-passing state) with only the
version bump to 3.9.1.

Co-Authored-By: Claude Opus 4.6 (1M context) <noreply@anthropic.com>

* fix(ci): regenerate package-lock.json with npm 10 for stable npm ci

npm 11 (Node 24) produces internally inconsistent lock files that npm ci
rejects — each npm install resolves transitive deps differently. Generated
with npm 10 (Node 22 LTS) which produces stable lock files compatible with
both npm 10 and npm 11.

Dragan Spiridonov 4e513b431d fix: resolve 3 P0 blockers and CI failures from QE v3.8.13 audit
- Delete dead MCP server.ts (911 lines, never used in production — entry.ts
  imports MCPProtocolServer). Add legacy aliases in index.ts for backwards compat.
- Fix ESLint: rename .eslintrc.js to .eslintrc.cjs (CommonJS in ESM project)
- Fix hardcoded version '3.0.0' in protocol-server.ts and http-server.ts —
  now reads dynamically from package.json
- Fix vitest process hang: add worker-level afterAll force-exit (setup-worker.ts)
  and global teardown safety net. Native modules (better-sqlite3, hnswlib-node)
  held open handles preventing fork worker exit.
- Add test gate to npm-publish.yml — 194 unit test files must pass before publish
- Disable benchmark.yml release trigger (benchmarks/suite.ts doesn't exist)
- Remove continue-on-error from test steps in optimized-ci.yml (was masking failures)
- Add getToolDefinitions() to MCPProtocolServer for test introspection
- Update e2e and integration tests to use production server API

Dragan Spiridonov 505e2944ff fix(ci): ensure junit reporter runs for timeout recovery (#376)
Two fixes:
1. ci-vitest-run.sh now injects --reporter=junit before user args so

   vitest.config.ts reporters. Also use portable grep -o instead of

Dragan Spiridonov 3f479e4a6f fix(ci): handle vitest process hang after test completion (#376)
Vitest completes all tests but hangs due to open handles (SQLite
connections, HNSW models, timers). The `timeout 480` command kills
it with exit code 124, which CI treats as failure even though all
tests passed.

Add scripts/ci-vitest-run.sh wrapper that checks junit.xml for
actual test results when timeout kills the process. If all tests
passed, exit 0 instead of 124.

Fixes 6 CI jobs: Journey (Remaining Subdirs, Root-level, RuVector),
MCP (Integration, Handlers, Root Files).

Dragan Spiridonov 3efc40953c fix(ci): add OS-level timeout guards and fix hanging tests (#350)
Tests in 6 CI shards were hanging indefinitely in native code (HNSW,
QEReasoningBank, SQLite), causing GitHub to report them as "cancelled"
at the job timeout-minutes boundary with no diagnostic output.

Root cause: vitest's JS-level testTimeout (10s) cannot interrupt native
C++ code. When bank.initialize() or server.invoke() blocked in HNSW
index loading or cross-domain transfers, the forked process stalled
until the GitHub job timeout killed it silently.

Workflow changes (optimized-ci.yml, mcp-tools-test.yml):
- Wrap all vitest commands in `timeout 480` (8 min OS-level SIGTERM)
  so hangs produce a clear failure (exit 124) instead of silent cancel
- Add --reporter=verbose to identify which test was last running

Test fixes:
- mcp-server.test.ts: add Promise.race timeout guards to all domain
  tool invocations (coverage_analyze, security_scan, test_generate)
  and fleet_init beforeEach — these call real handlers that can hang
- rvf-production-wiring.test.ts: add withTimeout() guards around
  bank.initialize(), writer.initialize(), bank.dispose() which load
  HNSW indices and cross-domain transfers via native code

Verified: rvf-production-wiring previously hung indefinitely (killed
by timeout 120 locally); now completes in 907ms. MCP Root Files shard
(122 tests) completes in 10.7s.

Dragan Spiridonov 56adeba0e8 fix(ci): use npx vitest and add missing build steps in sharded jobs
All 8 sharded CI jobs (4 Journey + 4 MCP unit) failed with "vitest:

`npx vitest`. Vitest is a devDependency and not in PATH on CI runners.

Also adds missing `npm run build` step to 4 MCP unit test shards
(handlers, security, tools+transport, root) — other passing jobs
all had this step.

Dragan Spiridonov c1b17bfa5b fix(ci): split slow monolithic jobs into parallel shards and stop perma-failing schedules (#350)
The Optimized CI and MCP Tools Testing workflows were timing out because
single jobs ran 80+ test files sequentially, exceeding their 25-30 min
timeout-minutes limits. GitHub reports these as "cancelled", hiding the
real CI signal.

Changes:
- Split Optimized CI fast-tests (1 job, 25 min timeout) into 6 parallel
  jobs: 4 journey shards by directory (governance, ruvector+coordination,
  root-level, remaining subdirs) + contract tests + code intelligence
- Split MCP unit tests (1 sequential job, 30 min timeout) into 4 parallel
  shards: handlers, security, tools+transport+metrics, root files
- Fix concurrency groups: PRs cancel stale runs, main never cancels
  (was cancelling in-progress main runs on rapid successive pushes)
- Disable schedule triggers on 4 workflows with 100% failure rate:
  n8n-workflow-ci, sauce-demo-e2e, qcsd-production-trigger, benchmark
  (workflow_dispatch still available for manual runs)

Expected impact: CI wall-clock drops from 25+ min (timeout) to ~15 min,
and all checks now complete with actual pass/fail signal.

Dragan Spiridonov 23c3dcb730 fix(ci): resolve timeout-cancellations and broken contract test path (#350)
CI checks were being killed as CANCELLED due to timeout limits that
didn't account for npm ci + build overhead. Also fix test:contracts
which pointed at all integration tests instead of actual contract tests.

- Increase fast-tests timeout: 15 → 25 min (optimized-ci.yml)
- Increase MCP unit/integration test timeouts: 20 → 30 min (mcp-tools-test.yml)
- Fix test:contracts script to run tests/unit/domains/contract-testing/
- Fix CI guard to check correct contract test directory

Dragan Spiridonov cc9c742d1e fix(ci): resolve CI timeouts, cancellations, and test failures (#350)
- Add concurrency groups to Optimized CI, MCP Tools Testing, and Coherence
  workflows to cancel stale runs on new pushes
- Create focused test:journeys:ci script excluding heavy dirs (learning,
  domains, llm, mcp, sync) already covered by other CI jobs
- Increase Fast Tests timeout 10→15min, MCP Integration timeout 15→20min
- Fix check logic to catch cancelled/timed-out steps (!=success vs ==failure)
- Make Performance Gates and Coverage Analysis resilient to fast-tests failure
- Fix task-handlers unit tests (63/63 pass) by injecting mock queen when
  PersistentSONAEngine fails in memory-only mode

Dragan Spiridonov df992ef892 fix(ci): verify CLI/MCP bundles instead of raw ESM index.js
dist/index.js uses bare directory imports (e.g., './shared/types')
which Node.js ESM rejects with ERR_UNSUPPORTED_DIR_IMPORT. This is
fine for bundler consumers but fails with direct node import.

Verify the self-contained esbuild bundles (CLI + MCP) instead —
these are what users actually execute.

Dragan Spiridonov f040ee7ce6 fix(ci): replace test suite with import smoke test in publish workflow
The vitest forks pool hangs on GitHub's 2-core runners when test files
trigger HNSW/ONNX/SQLite initialization across parallel forks. This
affected even test:unit:fast because qe-reasoning-bank loads the full
HNSW stack.

Replace with fast import smoke tests:
- Verify dist/index.js imports cleanly (2s)
- Verify CLI bundle executes --version (2s)

The full 19K+ test suite runs in PR CI (optimized-ci.yml) before merge.
Running tests again at publish time is redundant and unreliable on
constrained runners. Build artifact verification is sufficient.

Dragan Spiridonov f5780ce55c fix(ci): use test:unit:fast for publish smoke tests
Root cause analysis of publish timeouts:
- v3.7.22 had 10.9K tests completing in 6min on CI runners
- v3.8.0 added 31 new test files (+9K tests) from RuVector integration
- The heavy test subset (coordination, integrations, domains) triggers
  HNSW/ONNX/SQLite initialization per forked process, which is ~8x slower
  on GitHub's 2-core runners vs our 4-core Codespace
- Tests would hang during parallel fork initialization, exhausting the
  30-minute job timeout

Fix: Run test:unit:fast (~7K core tests) as publish smoke tests.
The full suite (19K+ tests) is already validated by PR CI checks
(optimized-ci.yml) before merge — re-running in publish is redundant
and unreliable on constrained runners.

Dragan Spiridonov 4ab89962a9 fix(ci): use test:unit instead of test:ci in publish workflow
The full test:ci suite (~20K tests) exceeds GitHub runner time limits.
Unit tests (~15K tests) provide sufficient verification for publish —
the full integration suite already runs on PR checks before merge.

Dragan Spiridonov 1855a9f544 fix(ci): increase workflow timeouts for larger test suite
Build and Verify job in npm-publish.yml: 20m → 30m
MCP validation job in mcp-tools-test.yml: 5m → 10m

The RuVector integration added ~4K new tests, pushing the full
test:ci suite past the old 20-minute limit on GitHub runners.

Profa 072432d396 fix(ci): fix YAML syntax error in qcsd-production-trigger workflow
Replace unindented heredoc (<<EOF) with echo statements in the

Profa 2253d05618 feat: flatten project structure — archive v2, promote v3 to root
Remove the nested v3/ subdirectory by moving all active source code,
tests, configs, and assets directly to the project root. Archive v2/
(dead code, 847MB) to v2-archive.tar.gz.

Key changes:
- Move v3/src/ → src/, v3/tests/ → tests/, v3/packages/ → packages/
- Move v3/tsconfig.json, v3/vitest.config.ts to root
- Merge v3/package.json into root package.json (ESM, scripts, deps, exports)
- Merge v3/scripts/ into scripts/, v3/implementation/ into docs/implementation/
- Update all CI workflows to remove "cd v3 &&" and v3/ path prefixes
- Fix all init installer path resolution (3-level-up → 2-level-up)
- Fix hooks to use "node ./dist/cli/bundle.js" instead of bare "aqe"
- Update settings.json ADR/DDD directories to docs/implementation/
- Fix helper scripts (statusline, adr-compliance, ddd-tracker, guidance-hooks)
- Update 58 agent definitions: aqe/v3/ → aqe/ memory namespaces
- Update 15 guidance shards: v3/src/ → src/ path references
- Fix sync interfaces: remove v3/ and v2/ path references
- Fix test path calculations (process.cwd() + '..' no longer needed)
- Fix flaky perf test threshold (500ms → 1000ms for CI environments)
- Fix complexity-analyzer test to match ADR-051 keyword-based eligibility

Build passes, 17,901 tests pass, CLI and MCP bundles verified.

Profa f1169bc3a8 fix: resolve test timeouts, OOM crashes, and CI artifact failures (#294, #251)
- Mock createTaskExecutor in domain-handlers and wrapped-domain-handlers
  tests to eliminate 30s+ real execution timeouts
- Reduce fleet init churn in core-handlers tests from 31x to 5x using
  shared beforeAll/afterAll lifecycle
- Set vitest maxForks=1 and fileParallelism=false to prevent OOM in
  DevPod/Codespace (15GB total, ~7GB consumed by VS Code + Claude)
- Add actions:write permission to optimized-ci.yml and mcp-tools-test.yml
  to fix artifact upload 403 Forbidden errors
- Delegate root test:mcp script to v3's test:unit:mcp (respects exclusions)
- Widen topology-optimizer convergence tolerance (2x→3x) with retry:3
  to stabilize probabilistic TD error test

Lalitkumar Bhamare 8f52a1cc37 feat(qcsd): Production Telemetry Swarm — 5th QCSD lifecycle phase + auto-trigger (#271)
* feat(qcsd): add Production Telemetry swarm — 5th QCSD lifecycle phase

Complete the QCSD lifecycle (Ideation → Refinement → Development → CI/CD → Production)
with the Production Telemetry swarm for post-release health assessment.

- 12 agents (3 core + 7 conditional + 2 feedback) across 4 DDD domains
- 9-phase execution: flag detection, parallel core spawn, conditional spawn,
  HEALTHY/DEGRADED/CRITICAL decision logic, report generation, learning
  persistence, sequential feedback loop closure
- 7 domain flags driving conditional agent activation
- Cross-phase signal consumption from CI/CD via memory_query
- Dual feedback loops to Ideation (strategic) and Refinement (tactical)
- All scores normalized to 0-100 scale across all agent types
- Enterprise-integration agents (middleware, SAP, SoD) wired into decision logic
- Phase 8 sequential enforcement for data-dependent feedback agents
- JSON schema with 15 $defs including 3 enterprise analysis types
- 8 eval test cases including 3 decision logic determinism tests

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>

* fix(qcsd): align eval test case domain flags with provided metrics

tc006: add HAS_MIDDLEWARE (middlewareHealth: 12 present)
tc007: fix HAS_INFRASTRUCTURE_CHANGE true (resilienceScore: 92),
       add HAS_MIDDLEWARE (middlewareHealth: 88),
       add HAS_AUTHORIZATION (sodCompliance: 95)
tc008: add HAS_MIDDLEWARE (middlewareHealth: 55),
       add HAS_SAP_INTEGRATION (sapHealth: 40)

No test outcome changes — flags now match the non-null metrics
each test case provides, preventing future maintainer confusion.

* feat(qcsd): add automatic post-deploy telemetry collection and production swarm trigger

- New script: scripts/collect-production-telemetry.sh
  Collects DORA metrics (deployment frequency, lead time, change failure
  rate, MTTR) from GitHub API via gh CLI. Outputs structured JSON to
  docs/telemetry/production/. Runs standalone or in GHA.

- New workflow: .github/workflows/qcsd-production-trigger.yml
  Triggers after npm publish (workflow_run), weekly schedule, or manual
  dispatch. Collects telemetry, commits to repo, creates GitHub Issue
  with invocation command for the production swarm.

- Edit: .claude/hooks/cross-phase-memory.yaml
  Added Loop 5 (CI/CD → Production deploy trigger) with post-release-decision
  and pre-production hooks. Updated routing and header comment (4→5 loops).

- Edit: .claude/skills/qcsd-production-swarm/SKILL.md
  Added Step 0.5 (auto-detect pre-collected GHA telemetry). Non-breaking
  fallback to manual input when no telemetry file exists.

- New: docs/telemetry/production/.gitkeep (output directory)
