Abdul Wahab f775f9e585 Fix the three design-context findings from the command test pass 
- edit_request replies now apply attached --answers/--context through the
  session, as visual-cues.md's edit-loop contract already promised
- the bundle's per-file cap rises to 8 MiB so the chosen cue image survives
  export/import (the 20 MiB bundle ceiling stays)
- the questionnaire refuses to start without visual-cues/cues.json instead
  of serving a broken palette screen
