feat: dedupe session index, reconcile legacy outbox to zero
91 byte-proven duplicate Runs / 1,043 chunks removed across 24 sources
(including tonight's live ee2cabd9 incident, resolved exactly as
alerted). 977 outbox files reconciled through the canonical replay
tool: 911 redundant, 59 covered, 7 suffixes indexed; files retained by
the never-delete rule. Backup + SHA receipts kept; independent post-
pass check: integrity ok, zero overlap pairs.
