Marcus Webb 79bd3ded90 refactor: inline critical reference content into mcp-builder SKILL.md

Agents skip reference files and improvise. All critical-path content
(common errors, deploy checklist, testing pattern, cloud deployment,
production patterns, context features, CLI reference) is now inline
in SKILL.md. Only integration-patterns.md remains as a truly optional
reference for OpenAPI/FastAPI conversion use cases.

SKILL.md: 182 -> 446 lines. 7 reference files removed.
