The scale moved from 1-5 to 0-10 in #2438, but `5` stayed a legal value
and several surfaces kept printing a bare range. A caller habituated to
the old top-of-scale still answers `5`, which now records as a mediocre
5/10 instead of a clean run.
Every surface that asks for a rating now names the top end, from one
constant, and a bare bottom-half score with no comment gets a soft warn
that restates the scale.
