David Cramer 17d0614c55 Remove /getIssues command and issue-summarizer subagent 
These capabilities are being rebuilt as part of the getsentry/sentry-mcp
server. Also removes the now-redundant MCP-SETUP.md since MCP is
auto-configured via .mcp.json and plugin.json.
