Trevin Chow b52f92b757 fix(skills): remove cross-skill file references and add platform-variable fallback ...

ce-brainstorm and ce-plan handoff docs pointed at files inside the
ce-proof and ce-doc-review skill directories. Skills resolve paths from
the user's CWD and install under versioned cache paths, so cross-skill
file pointers break at runtime; the handoffs now name the target
skill's workflow semantically and let it load its own references.

ce-update assumed ${CLAUDE_SKILL_DIR} always resolves. On harnesses
that do not expose it, the skill now routes to its existing
not-a-marketplace-install handling instead of failing with a raw
script error.
