Trevin Chow 1ced66ec1c fix(ce-review): add recursion guard to reviewer subagent template ...

Reviewer subagents spawned by ce:review could re-enter the parent
review workflow via skill routing, causing nested review trees instead
of a single orchestrator pass. Adds the same leaf-reviewer guard
already applied to document-review in #523.

Closes #526
