Trevin Chow 0fb946350b fix(simplify-code): restore context.mjs dispatch setup

PR #1311 folded two changes together: the prompt-size reduction and a
migration off the shared skill-context setup script. This restores the
dispatch mechanism while keeping the diet: scripts/context.mjs returns
byte-identical to the shared copy, the SKILL.md Setup fence and original
Step 2 dispatch wording are back, the skill rejoins DISPATCH_SKILLS in
the parity test, and the skill guide drops the paragraph documenting the
inline replacement semantics.
