- Restore chat-numbered-list fallback for the 5-option overflow menu when
  the platform's blocking question tool is unavailable or errors (Codex
  edit modes, Pi `ask_user` no-match), matching the AGENTS.md mandate to
  never silently skip a blocking question. Mirrored in SKILL.md.
