Extends Principle 4 of the agent-friendly-CLI rubric with the case behind
#1607: a "not found" path that serves both a genuine read error and a caller
correctly discovering there is nothing there, and the follow-on trap that
splitting one error into several outcomes re-creates the conflation whenever a
branch borrows another's exit code.

Adds `Clean skip` to CONCEPTS.md — the outcome class the whole distinction
turns on. Quotes the doc's pre-existing unquoted `title:` so its frontmatter
parses under strict YAML.
