Trevin Chow a5832cc5f6 fix(ce-work): require review receipt or explicit skip before shipping

Standalone ce-work could finish implement→push without ever invoking
ce-code-review because the mandate was prose-only and the mechanical
escape hatch was self-classified. Add an always-loaded completion gate
(receipt or fixed skip phrase), re-check at the commit/PR handoff,
tighten mechanical exclusions, and pin the contract in tests.
