Cursor Agent 3f94834661 fix(ce-code-review): apply the protected-subject veto in the report leaf ...

After #1692, Stage 5b step 5 runs in the report leaf from validator-outcome.json.
An infrastructure-failure outcome must take the unresolved report-only route
for protected and P0/P1 findings rather than the old boolean drop, and later
passes must not pull an unresolved gate back into the actionable queue.
