Trevin Chow 5bbb0e1dfd refactor(plugin): make plugin skills-only and root-native ...

Flatten the plugin to the repo root, remove standalone agents (43 -> 0),
retire 12 standalone skills, and move specialist behavior into skill-local
prompt assets under references/agents and references/personas.

Integrate main's ce-code-review deletion-signal coverage and ce-simplify-code
simplification safety hardening (#969) while preserving agentless local-prompt
naming in review output sections.
