Trevin Chow c102887f0d fix(ce-doc-review): scope the interactive references and pin the apply-path HTML rule ...

Two review findings:

- Codex: the Phases 3-5 pointer lost the interactive-mode condition, so a
  non-interactive run would load walkthrough.md and bulk-preview.md — question
  menus and interactive mutation paths that mode never takes. The condition is
  back, and a non-interactive run stops at the synthesis envelope.
- Cursor: the HTML guard pinned the ID-bearing sibling-mirror rule only where
  it is decided (intake), not where it fires (fix application in synthesis).
  It now asserts both.
