Trevin Chow 3ea41a5f93 fix(document-review): show contextual next-step in Phase 5 menu ...

The "Review complete" option in document-review's interactive menu had a
vague description ("document is ready") that the model would embellish
with incorrect caller context (e.g., "proceed to brainstorm handoff"
when the actual next step is planning). Now uses the document type
classification from Phase 1 to show the concrete next step:
requirements docs → "Create technical plan with ce:plan",
plan docs → "Implement with ce:work".
