Trevin Chow ac03b46406 fix(resolve-pr-feedback): serialize same-file dispatch across clusters and individuals ...

Conflict avoidance now applies across all dispatch units, not just
within individual dispatch. If a cluster and an individual thread both
touch the same file, they are serialized to prevent parallel edit
conflicts.
