Trevin Chow 69f52aaad7 fix(review): address PR feedback on notification and skip paths ...

- Gate "Link to PR" notification on whether a PR was created (ce:work,
  ce:work-beta) so commit-only path doesn't require a nonexistent URL
- Remove "(or is skipped)" from ce:plan Phase 5.4, closing the bypass
  path for mandatory document-review
