Kieran Klaassen 15d8011e87 fix(review): apply review findings ...

Corrects the plan's count arithmetic to the shipped 27->28 (it assumed the
unmerged ce-sweep baseline), tightens the routing-test separator regex so
mid-word hyphens cannot satisfy the action match, pins the artifact filename
at the Phase 4 write site to match destinations.md, and adds platform
option-cap fallback language to the Phase 6 destination ask.
